Job Summary:

Carease Health - NJ is seeking an experienced Nurse Practitioner (NP) or Physician Assistant (PA) with a background in Geriatrics or Internal Medicine to deliver high-quality preventative care to residents in skilled nursing facilities. This hybrid role combines on-site visits with remote work from home, supported by a dedicated scribe team for efficient documentation.

Responsibilities:

  • Provide high-quality preventative care to residents in skilled nursing facilities

  • Conduct on-site visits and remote work from home

  • Build strong relationships with facility staff and maintain a patient-centered approach

  • Collaborate with a dedicated scribe team for efficient documentation

  • Ensure compliance with facility processes and protocols

  • Participate in quality improvement initiatives to minimize hospitalizations and elevate patient care standards

Requirements:

  • Active NP or PA license (in the state of practice)

  • 1+ year of experience in Geriatric care and documentation review

  • Excellent communication, collaboration, and teamwork skills
