About the Role: As a key member of our clinical team, you will provide on-site visits and remote work from home, supported by a dedicated scribe team for efficient documentation. Our goal is to minimize hospitalizations, elevate patient care standards, and increase reimbursement for your facility. Responsibilities: Provide comprehensive geriatric or internal medicine care to residents in skilled nursing facilities Conduct on-site visits and remote work from home, with support from a dedicated scribe team Build strong relationships with facility staff and maintain a patient-centered approach Collaborate with other healthcare professionals to ensure seamless care transitions and coordination Develop and implement individualized care plans for patients Monitor patient progress and adjust treatment plans as needed Requirements: Active NP or PA license in the state of practice 1+ year of experience in Geriatric care and documentation review Excellent communication, collaboration, and teamwork skills Masters degree in Nursing or related field What We Offer: Hybrid Role!
