Description Position Summary: Provides clinical knowledge and expertise in coordinating patient care in the Grace Cottage Family Health setting as directed by the medical provider. Qualifications: Education/ Experience: Certification per American Association of Medical Assistants preferred (AHA) , LPN License, or LNA License. Other Requirements: Proficient computer skills required. Experience working with an electronic health record preferred. Requirements Essential Functions Of The Position: Treats patients and their families with respect and dignity. Facilitates patient flow throughout the encounter. Completes chart prep, driven by visit type and chief complaint including reviewing the medical chart for current information regarding the patient’s medications, test results, hospital records, operative reports, notes from consulting specialists, etc. Any missing results or other information should be requested and obtained. Maintains and updates clinical information including identifying and documenting active problems, completing/resolving historical problems, updating social and family history, reviewing and updating allergy list, reconciling patient’s current medication list, and determining if the patient needs refills after documenting any reported side effects of medications. Records chief complaint for visit, as obtained through patient interview.
