Job Description

Job Summary: The Clinical Nurse C-Inpatient utilizes skilled discretion and independent judgment to prioritize, coordinate, provide and document direct/indirect age-specific and developmentally appropriate quality patient-centered care in a fast-paced environment. Performs the nursing process (assessment, care planning, intervention, evaluation) independently and in accordance with LLUCH’s established Professional Practice Model (Duffy's Quality Caring Theory) to achieve proficient individualized physical, psychosocial and spiritual wellness and provide compassionate bereavement support. Meets qualifications, licensure, certifications and competencies as defined in the unit/department-specific Plan for Providing Care. Functions in a skills mix environment and effectively supervises and delegates tasks to a team of licensed and unlicensed staff in accordance with level of competency. Skillfully resolves disagreements/conflicts among team members as needed. Applies theoretical concepts to clinical practice and exhibits self-motivation to continually expand knowledge base and clinical skills with a focus in area of specialty and patient population served. Inspires and adopts the transformation of evidence into bed-side practice to optimize patient outcomes. Functions within the expectations of an accomplished professional nurse and meets nursing excellence standards to achieve and maintain Magnet Recognition as outlined by the American Nurses Credentialing Center. Represents nursing as an empowered profession and readily embraces new knowledge, innovations and improvements. Provides a positive influence to facilitate change and optimize patient satisfaction and outcomes. Engages in opportunities to directly and/or indirectly influence decision-making for bedside clinical practice by supporting and/or participating in professional/shared governance councils, committees, task forces and staff meetings. Exhibits refined interpersonal skills that portray professionalism and maximize excellence in customer service and safe patient care. Contributes to a work environment of caring and cooperation among a culturally diverse workforce and patient population. May be required to participate in on-call rotation based on department business needs. Performs other duties as needed.

Education and Experience: Bachelor of Science Degree in Nursing (BSN) required. If hired into the CN-C position prior to April 1, 2020, an Associate Degree or Diploma Degree in Nursing required. Minimum of two years' experience required. External hires into the CN-C position are granted one year to meet CN-C criteria outlined in the Clinical Nurse Ladder handbook. The CN-C is eligible to be promoted into the CN-D classification when criteria outlined in the Clinical Nurse Ladder handbook and job specifications outlined in CN-D job description are met.

Knowledge and Skills: Demonstrates advanced competencies in clinical nursing. Ability to apply theoretical concepts to clinical practice and continually expands knowledge base and clinical skills with a focus in area of specialty and population served. Basic knowledge of healthcare regulations and accreditation required. Exhibits professional behavior at all times and works calmly and responds courteously when under pressure. Able to: speak, read and write legibly in English (and Spanish preferred) with professional quality; use computer, printer and software programs necessary to the position, e.g., Word, Excel, Outlook, PowerPoint, electronic medical record, electronic event reporting program; troubleshoot and calibrate patient care equipment; perform technical patient care activities; relate and communicate positively, effectively and professionally with others; be assertive and consistent in following and/or enforcing policies; lead, supervise, teach and collaborate; accept direction; think critically; work independently with minimal supervision; perform basic math functions; manage multiple assignments effectively; organize and prioritize workload; problem solve; recall information with accuracy; pay close attention to detail; distinguish colors and smells as necessary for patient care; discern temperature variances through touch; hear sufficiently for general conversation in person and on the telephone; identify and distinguish various sounds associated with the work place; see adequately to read computer screens, medical records and written documents necessary to position.

Licensures and Certifications: Active California Registered Nurse (RN) license required. Basic Life Support (BLS) certification issued by the American Heart Association required. Other certifications required as listed in the department-specific Plan for Providing Care.

Home Health Weekend Registered Nurse RN

The RN would be on call from Friday at 4:30 pm until Monday at 8:00 am (eastern). The RN would be responsible to cover any as-needed visits that arise, as well as scheduled weekend visits including admission visits. Compensation will include an on-call stipend PLUS pay per visit for each visit completed. The Nurse will be responsible for assessing patient health problems and needs, developing and implementing nursing care plans, and maintaining medical records. May advise patients on health maintenance and disease prevention or provide case management. Licensing or registration is required. ESSENTIAL DUTIES AND RESPONSIBILITIES The essential functions include, but are not limited to the following: On call from Friday at 4:30 pm until Monday at 8:00 am (Eastern) Must cover any as-needed visits that arise, as well as schedule weekend visits, including admission visits Provide skilled nursing treatments as ordered by the client’s doctor Complete evaluation tasks, including vital signs and medication reviews Coordinate with physicians, therapists, and other individuals in the client’s care plan Assess and chart observations of the client’s condition at each visit Maintain complete and accurate documentation of nursing care Work collaboratively with all members of the healthcare team Provide education to clients and families on proper home health care procedures and strategies Record patients' medical information and vital signs Monitor, record, and report symptoms or changes in patient's conditions Administer local, inhalation, intravenous, or another anesthetic Prepare patients for and assist with examinations or treatments Modify patient treatment plans as indicated by patients' responses and conditions Order, interpret, and evaluate diagnostic tests to identify and assess patient's condition Monitor all aspects of patient care, including diet and physical activity Engage in research activities and participation in formal educational training activities related to nursing Perform other duties as assigned MINIMUM QUALIFICATIONS (KNOWLEDGE, SKILLS, AND ABILITIES) Must be available on weekends from Friday 4:30 pm until Monday 8:00 am (Eastern) Associates degree required; Graduate of an accredited state-approved, Bachelor’s Degree from a school of nursing preferred Current and valid California Registered Nurse License Prior nursing experience, specifically in home health (preferred) Current CPR certification (required) Driver’s License Car Insurance Car Registration Strong interpersonal skills that build personal rapport and establish trust with the client Computer literate Strong organizational skills Excellent written and communication skills Ability to meet physical demands of the job (ex: lifting 50 pounds for transfers) Home health care experience Bilingual (Spanish) preferred PHYSICAL DEMANDS AND WORK ENVIRONMENT The physical demands described here are representative of those that must be met by an employee to perform the essential functions of this position successfully.
