Responsibilities

Patient Care & Advocacy

    • Facilitate, implement, and evaluate individualized Patient Plans of Care and corresponding outcomes.
    • Collaborate with the healthcare team to support positive patient results and ensure continuity of care.
    • Serve as a strong patient advocate, promoting safety, dignity, and well‑being.

Clinical & Technical Competence

    • Deliver timely and effective nursing interventions based on patient needs.
    • Demonstrate strong clinical skills and technical proficiency in all aspects of patient care.
    • Utilize sound clinical judgment to assess, analyze, and communicate patient information promptly.

Documentation & Communication

    • Accurately document assessments, interventions, patient responses, and provider communications in real time.
    • Perform ongoing review, initiation, and implementation of provider orders each shift.

Patient Safety & Risk Assessment

    • Identify patient safety risk factors, including fall risk, skin integrity concerns, and the need for restraints.
    • Implement appropriate safety interventions promptly and consistently.
    • Follow medication safety policies and complete all required Safety Tracking Tools.

Delegation & Team Leadership

    • Delegate tasks appropriately and supervise assigned staff to ensure high‑quality patient care.
    • Take responsibility for all care provided to assigned patients.
    • Report performance concerns to maintain a safe, supportive, and effective care environment.

Patient & Family Education

    • Assess educational needs of patients and families, providing clear written and verbal instructions.

Qualifications

 

  • 2 years nursing experience caring for Step Down level of care patients including cardiothoracic surgery patients, preferred.
  • EPIC preferred

  • NIH - Stroke Certification

Professional nursing practices, requiring specialized knowledge judgment and skill are performed using the process in accordance with hospital policy, procedures and nursing philosophy. Responsibilities: Provide direct, professional nursing care to specified population of patients in the acute care setting. Develops a patient centered plan of care based on individualized needs. Maintains confidentiality of the patient medical record. Counsel and educate patients and their care partner on treatment plans. Administer medication following the five rights of medication administration. Provide wound care and dressing changes, as applicable. Read and interpret patient information, making treatment decisions, when appropriate. Consult with physicians to decide on best treatment decisions when appropriate Performs nursing interventions to achieve optimal patient outcomes. Observe and record patient behavior and response to nursing interventions. Demonstrates quality, safe and effective patient care. Provides delegation to unlicensed assistive staff and Licensed Practical Nurse (LPN) staff Demonstrates standards of excellence which includes practicing nursing with core values of compassion, accountability, respect, teamwork and integrity. Demonstrates understanding and executes quality and performance improvement plans. (only for RN evaluation) Participates in decision making and takes actions based on ethical principles.
