Under the supervision of the Wound Care Manager, assesses, plans, implements, and evaluates care of patients with wounds, ostomies and continence disorders. This includes complex draining wounds, fistula, pressure ulcers, incontinence, and ostomies. The Wound Ostomy Continence Nurse ensures prompt, efficient, and effective patient care and appropriate delegation of related clerical and program support activities.

Responsibilities

Supports the Wound, Ostomy, Continence Care Manager in the development and implementation of the WOCN Program to meet the needs of patients and community.
Guides and collaborates with the nursing staff, medical staff, patient, family and other members of the health team to assess, plan, implement, evaluate, and improve the nursing needs of patients with Wounds, Ostomies, fistulas, drains or altered skin integrity including physical, emotional and spiritual needs.
Assesses the nursing and medical staff’s educational needs and collaborates with the Wound, Ostomy, Continence Care Manager to develop, implement, and conduct a variety of teaching programs regarding WOCN needs. Provides resources to clinical preceptors to enhance the learning process during clinical orientation/rotation.
Assesses, plans, implements, and evaluates the plan of care for continuity of care in collaboration with the medical staff, nursing staff, social worker, and other health care teams, including care after discharge. Evaluates patients’ response to treatments and initiates alternative treatment approach, when indicated.
Assesses physical, emotional and social factors that may affect the patient’s rehabilitation.
Functions as a consultant to the staff for patients with complex draining wounds, fistulas, pressure ulcers, and skin problems related to fecal and urinary incontinence.
Enhances professional growth and development through participation in educational programs, nursing leadership meetings, hospital-wide committee (Infection Control and QI), and maintaining high level of competence in WOCN specialty.
Verbalizes and/or demonstrates knowledge of clinical information and practices essential to provide age-specific care for the infants through the geriatric population.
Develops and administers departmental standards of performance, policies and procedures.
Assists in making purchasing recommendations for ostomy and wound care supplies.
Assists in planning and implementing program, staffing, supervision and program development.
Assists in developing and implementing a program of professional and community contacts.
Assists in developing and implementing professional growth activities for all staff.
Maintains an effective liaison with physicians and other hospital personnel.
Maintains open lines of communication between Department Staff, Administration and other departments of the hospital.
Acts as a resource for staff regarding the contents of applicable professional codes, SRRMC/Tenet employee handbooks, and departmental P&P and hospital policies.
Maintains current competence and skill levels required for work areas.
Adheres to policies and practices of applicable professional organizations.
Demonstrates knowledge of criteria/accreditation standards (JCAHO, CARF, CHFA, ETC) and assists with survey preparation as assigned.
Assists the Wound, Ostomy, Continence Care Manager in orientation of new staff.
Maintains accurate records, reports and statistics as assigned.
Cooperates with other personnel in a positive manner to achieve departmental and interdepartmental objectives and maintains good employee relations.
Demonstrates responsibility for cost containment, conservation of supplies and equipment, as well as other organizational resources.
Demonstrates sensitivity to patient’s comfort and privacy. Adapts approaches based on recognized cultural and religious diversity.
Participates in Pre-Admission screening as required and completes according to policy and procedures.

Qualifications

Minimum Education:

Graduate of an accredited RN Program
BSN and Wound, Ostomy and Continence Certification, and experience preferred

Minimum Experience/Skills:

Minimum of one (1) years experience in an acute clinical nursing department, ideally a medical surgical or specialty area
Minimum of one (1) year experience as Wound, Ostomy, and Continence Nurse preferred
Experience where leadership, integrity, initiative and effective communication skills have been demonstrated
Must have knowledge of adult education theory

Licenses/Certificates/Credentials:

Current California RN license
Current BLS certification for Healthcare Providers (AHA certified)
Completion of an accredited Wound, Ostomy, Continence Training Program preferred Current Certification in Wound, Ostomy, and Continence preferred. The candidate must get the wound, Ostomy certification within 12 or 18 months of hire.

Responsibilities % of time Essential Function (Yes/No) Key Responsibilities (To be completed by Supervisor) 5 yes Coordinates and monitors the patient care delivery system to assure quality care, and compliance with internal policy, external regulations and accepted practices across the continuum of care 5 yes Partners with managers across services (eg:outpatient, children's, cancer center, etc to implement policies and procedures for coordination of care for all patient populations , 5 Represents the department / unit on interdisciplinary committees on a range of patient care topics, such as patient satisfaction levels, workforce quality improvement initiatives, workflow efficiencies and clinical information systems 5 Provides mentorship / leadership to new and emerging nurse leaders to smooth the transition from clinical roles to formal leadership roles 5 Educates on the business side of healthcare, while maintaining the care side 5 Reviews and approves individual leadership development plans and assists with identifying resources 5 Manages a unit in a cost effective manner, consistent with the financial goals of the organization 5 Plans and is accountable for the annual operating and capital budget for areas of responsibility 5 Develops staff in collaborative decision making and shared governance 5 Creates an environment for a shared voice in nursing practice and the exchange of ideas to continually improve nursing and patient care outcomes 5 Responsible for employee recruitment, performance evaluation, progressive discipline, and staff development 5 Develops a strong culture of teamwork to ensure the quality of daily operations, effective interdepartmental collaborations, and the timely achievement of objectives 5 Works with clinical educators and leadership to evaluate core competencies in nursing care for the unit 5 Works with IT managers to plan and implement new or improved patient care information systems 5 Determines staffing needs and plans 5 Monitors performance of nursing units and nursing services 5 Coordinates activities with other units and departments regarding patient care 5 Works collaboratively with labor relations on bargaining unit issues and contract negotiations at the local level 5 Facilitates and oversees resolution of human resources, labor relations, employee safety and risk management issues Qualifications Required qualifications: Minimum of five (5) years’ experience in an academic medical center as a circulating and scrub nurse. Demonstrated hospital management skills, with progressive expertise in patient care services. Knowledge to manage disaster incidents, critical events and ability to problem solve with critical thinking skills. Strong interpersonal skills to create and maintain a climate for effective communication and collaborative decision making. Strong leadership, fiscal and operations management skills, with the ability to collaborate effectively with physicians, management and staff from all disciplines. Ability to create and maintain a goal-oriented climate of teamwork, collaboration and consistent achievement of objectives. Knowledge of relevant healthcare information technology and patient care workflows and processes. Bachelors or Master's Degree in Nursing required. Preferred Qualifications Master's Degree in Nursing or relevant area preferred. Licenses/Certifications Bachelors or Master's Degree in Nursing required. Registered Nurse in the State of California.
