Job Description

Provides physician-supervised physical and emotional care for the mother during labor, delivery, and postpartum phases. Evaluates progress and manages the labor and delivery. Evaluates and provides immediate care for the normal newborn.

Job Responsibility

  • Performs complete history and physical examination on new patients admitted to the labor room, and writes admission notes utilizing the attending physician when appropriate.
  • Discusses new admissions with the attending physician and nursing staff including noted complications.
  • Decides who will follow the patient and develops a plan for care.
  • Performs admissions diagnostics and care including, but not limited to pelvic examination, cervical cultures, drawing bloods, and starting intravenous lines.
  • Follows progress of labor of all patients in labor and delivery suite in collaboration with the attending physician and nursing staff.
  • Monitors fetal heart rate patterns and notifies the appropriate physician of significant deviations.
  • Examines patients periodically to determine progress in labor with ongoing assessment of maternal and fetal reaction to the progress of labor.
  • Reports any abnormalities in patient’s condition to physician including, but not limited to meconium, bleeding, hypertonic labor, fetal distress, failure of progress in labor, malpresentation.
  • Participates in deliveries.

Job Qualification

  • Bachelor’s Degree in Nursing and Master’s Degree in Nurse Midwifery from an accredited midwifery educational program, required
  • Current license to practice as a Registered Professional Nurse in New York State, required and current licensure as a Nurse Midwife in the State of New York, required.
  • American Midwifery Board Certification, required.
  • Has a collaborative practice agreement with a physician and designated protocols, both filed with the New York State Department of Education.
  • Experience preferred

The midwife will: Coordinate prenatal care to the caseload of patients in the ambulatory care setting. 2. Take pertinent history. Perform physical & pelvic examination. Obtain laboratory and diagnostic tests. Provide counseling & teaching as needed. 3. Identify risk factors and develop appropriate plan of management. Manage the care of selected patients admitted to the inpatient unit-based on unit protocols and with the collaboration of the attending physician. 4. Obtain physician consultation for plan of management for any medical or obstetrical complications not addressed in departmental protocols. Performs functions as identified in the Midwife Scope of Practice. 5. Provide Intra-partum Care as follows: Evaluate the status of labor and admits patient to the labor floor. Perform admission physical examination. 6. Perform venipuncture and starts intravenous infusions if nursing staff is unavailable. Obtain laboratory and diagnostic tests & evaluates results. Evaluate the progress of labor by vaginal examination. Make decisions regarding ambulation of the patient during labor. Request conduction anesthesia. 7. Perform amniotomy as indicated. Apply external fetal monitor & tocodynamometer. Apply internal fetal scalp electrode & pressure catheter. Evaluate fetal monitor tracing & consults with physician if abnormalities are present. Make decisions regarding the position(s) of the patient during second stage of labor. Make decisions regarding performance of delivery in the birthing room or transfer to the delivery room. 8. Utilize local infiltration or pudendal block. Perform & repairs episiotomies. Conduct spontaneous deliveries of cephalic presentations. Obtain specimen of cord blood for cord gases when appropriate. Manage the third stage of labor. Perform cervical & vaginal inspection. Repair minor vaginal & perineal lacerations. Take responsibility for the management of the uncomplicated patient in recovery room. 9. Decide when the normal infant should be transferred to the nursery. Request attendance of pediatrician at delivery when neonatal problems are anticipated. Order intermittent fetal monitoring based on patients risk status. Perform immediate appraisal of the newborn. Perform newborn physical examination. Sign birth certificate of delivery (operative delivery requires physician signature). Coordinate patient care in triage room on L&D. Attend and participate in L&D board rounds. 10. Participate in the care of patients during labor & delivery in consultation with attending physician: Perform prostaglandin insertions for cervical ripening. Manage Oct(s) after evaluation of the patient by the physician. Insert intrauterine pressure catheters. Perform & monitor amnio infusion. Monitor patient on IV oxytocin & regulates oxytocin infusion for induction or stimulation of labor following the initial stabilization & monitoring of the patient by the physician. Conduct spontaneous deliveries of these patients under supervision of the attending. 11. Manage the labor & conduct the deliveries of patients who have been approved by the physician for vaginal birth after cesarean section (VBAC). Repair cervical or extended vaginal lacerations under the supervision of the attending. Repair third & fourth degree extensions under the direct supervision of the attending. Participate as first assist on C-sections and/or BTL. Insert Foley catheter bulb for induction of labor. Perform misoprostol (Cytotec) insertions for labor induction. Order appropriate drugs to treat hemorrhage during third stage of labor per department policy for Post-Partum Hemorrhage. Manually remove placenta. Perform exploration of uterus. Perform selected patient high risk delivery at discretion and under the supervision of the physician. 12. Provide postpartum care as follows: Make daily rounds, examine patients, orders & interpret laboratory results, diagnostic tests & monitor the postpartum course. Provide postpartum teaching & counseling. Prepare patients for postpartum bilateral tubal ligations. Manage patients who have had cesarean sections or postpartum bilateral tubal ligations based on unit protocols & under the supervision of the attending physician. 1) Write medication & non-medication orders 2) Remove surgical sutures & staples Perform fever work-ups for patients with postpartum febrile morbidity, as per unit protocols & consults the physician for management assistance. Certified Nurse Midwife

Overview ANNUAL SALARY: $160,000.00 Position Summary: The Certified Nurse Midwife (CNM) provides comprehensive women’s healthcare across the lifespan, including prenatal, intrapartum, postpartum, and gynecologic care. This role includes both outpatient clinic responsibilities and inpatient hospital-based labor and delivery services. The CNM will provide inpatient care to patients at John Muir Medical Center with a varied number of days per week as needed, nights, weekends and some holiday shifts included. The CNM may also perform 1-4 clinical sessions in the outpatient setting at Ob-Gyn Specialist’s per week as needed. The CNM works collaboratively within a multidisciplinary team consisting of OB-GYN physicians, nurse practitioners, physician assistants, and support staff to deliver high-quality, patient-centered care. Responsibilities Key Responsibilities: Outpatient Clinic Care Provide routine and comprehensive gynecologic care, including annual exams, family planning, contraceptive management, and menopause care Manage prenatal patients, including initial OB visits, routine prenatal care, and patient education Perform postpartum evaluations and follow-up visits Perform limited sonography; first trimester dating and viability assessment Order, perform, and interpret diagnostic tests and screenings (e.g., labs, ultrasounds, Pap smears) Provide patient education on pregnancy, childbirth, breastfeeding, and overall women’s health Collaborate with supervising physicians for consultation and referral of high-risk patients Inpatient Labor & Delivery Care (Hospital-Based) Manage prenatal, intrapartum, and postpartum care in the hospital setting in accordance with standardized procedures and protocols and in consultation with a physician Perform vaginal deliveries independently for appropriate low-risk patients Assist physicians with operative deliveries as required. C-section surgical assist certification must be successfully completed as per hospital requirements Conduct labor assessments and monitor maternal and fetal well-being Provide triage evaluation and management of obstetric patients Perform procedures within scope, including: Amniotomy Episiotomy and repair Perineal laceration repair Collaborate closely with OB-GYN physicians for escalations and surgical interventions (e.g., C-section needs) Provide immediate postpartum care for birthing patients Perform hospital rounds, admissions, discharges, and inpatient documentation Group Care & Patient Education (Centering Program) Actively participate in the development, implementation, and ongoing enhancement of Centering Pregnancy (or group prenatal/postpartum care) classes. Collaborate with physicians, midwives, and clinical leadership to design evidence-based group care curriculum, workflows, and patient education materials. Serve as a facilitator for Centering classes, promoting patient engagement, peer support, and culturally competent care. Integrate clinical care, assessment, and education within a group setting while maintaining high standards of safety and patient-centered care. Assist with scheduling, coordination, and logistics of group sessions, ensuring efficient workflow and optimal patient participation. Support outreach and patient recruitment efforts to grow and sustain the Centering program. Collaboration & Care Coordination Consult and co-manage patients with physicians as appropriate Communicate effectively with nursing staff, hospital teams, and support staff Ensure smooth transitions of care between clinic and hospital settings Participate in Documentation & Compliance Maintain accurate, timely, and complete medical records in the EMR Comply with all regulatory, credentialing, and hospital privilege requirements including hospital medical staff bylaws and clinic policies. Adhere to evidence-based clinical guidelines and organizational protocols Participate in quality improvement, peer review, and maternal safety initiatives On-Call & Scheduling Expectations Participate in a call rotation for labor and delivery coverage Provide coverage for hospital deliveries, including nights, weekends, and holidays as required per contract agreement Maintain flexibility to support clinic and hospital demands Qualifications Qualifications: Education & Certification Certified Nurse Midwife (CNM) certification through AMCB Current, unrestricted California RN and CNM license issued by the Board of Registered Nursing Master’s or Doctorate degree in Nurse-Midwifery California Nurse-Midwife Furnishing Number Active DEA license (or eligibility to obtain) Eligibility for hospital credentialing and admitting / delivery privileges (if applicable) Licensure & Requirements Furnishing license (if applicable in CA) DEA registration Credentialing and hospital privileges at affiliated hospital(s) (e.g., John Muir) Preferred Prior clinical experience in women’s health, obstetrics, or labor & delivery preferred Experience managing low-risk pregnancies and deliveries strongly preferred Core Competencies Strong clinical judgment and critical thinking Excellent communication and patient education skills Ability to work collaboratively within a team-based care model Commitment to patient-centered, culturally competent care Flexibility and adaptability in a fast-paced clinical and hospital environment
