Responsibilities of the Registered Nurse include but are not limited to:

  • Ensures proper resident care is maintained in a clean and safe environment.
  • Develop and ensure the nursing care plan and collaborate with physicians, therapists, and nursing assistants
  • Makes nursing diagnoses, which serve as the basis for the strategy of care
  • Collaborates with the resident, members of the healthcare team
  • Records the administration of medication
  • Monitor assigned staff to ensure established safety regulations are followed and that all tasks are performed following all state and government regulations and safety procedures as outlined by the facility
  • Follows procedures related to the use of nursing equipment and supplies
  • Performs other duties as directed

 

Qualifications of the Registered Nurse include:

  • An active RN license
  • Prior experience as an RN in a nursing community
  • The desire to care for residents and act with integrity
  • The ability to work flexible hours

MDS Resource Nurse

Position Description We are looking for an outstanding MDS nurse to work closely with our facilities in NorCal- Stockton and Sacramento. This consultant will be responsible for providing expertise, leadership, modeling, and support for organization-wide initiatives. Provide education as necessary. · Interpreting rules, regulations and coverage guidelines and acting as primary resource for problem solving regarding care-based reimbursement systems and quality reporting program. · Provide consultative support and training to MDS Coordinators within the assigned facilities, to coordinate and guide resident-centered care. · Assist the Interdisciplinary Team with Care Planning. · Provide additional guidance related to clinical documentation and coding as it relates to the Resident Assessment Instrument (RAI). · Provide support and guidance to assist assigned facilities to achieve and maintain 5-Star Quality Measure Rating. · Auditing and monitoring of RAI timeliness and accuracy of information in furtherance of regulatory standards. · Support resident care by identifying trends and developing processes and action plans. Must be able to perform and present Root-Cause-Analysis. · Reviews and audits data to support assigned facilities’ obligation to accurately capture diagnosis codes in accordance with PDPM guidelines. · Provide on-going education, support, and make recommendations to optimize quality of care delivery. · Attend and support assigned facilities’ Quality Assurance Performance Improvement Programs (QAPI). · Work as an advocate for MDS Coordinators in relationship to the facility leadership, and the Interdisciplinary Team (IDT). · Maintain the most current knowledge of State and Federal guidelines and regulations related to the RAI and reimbursement. · The Regional MDS Nurse will assist the facility to prepare for the surveys and should be available to assist during the survey. Clinical Resource Nurse

We are seeking an experienced Clinical Resource Nurse, licensed as an RN in California who will partner with our clinical leaders of our facilities in California. The ideal candidate must possess at least five years of experience as a Director of Nursing for Skilled Nursing Care facilities, have a strong understanding of CMS and CDPH regulatory requirements, possess the capacity to lead staff and be willing to travel amongst the assigned facilities on a frequent basis. The primary purpose of your job position is to coach the Directors of Nursing in your region toward clinical, cultural, and financial excellence through intentional leadership. You will guide the teams to care for their residents and staff not only to meet the regulatory requirements but assist to refine processes that help the teams become the best care givers they can be. This job will require constant presence in the facilities and will not be remote. Qualifications Education: · Graduate of accredited school of nursing, BSN or MSN preferred. Licenses/Certifications: · Valid RN license in CA. · Valid CPR certification. · Valid driver’s license.
