How you’ll make an impact in this role

  • Lead Patient Care: Take ownership of the patient journey by implementing, monitoring, and adapting comprehensive care plans, ensuring the highest level of direct nursing care.
  • Coordinate the Team: Serve as the central coordinator, connecting and guiding all clinical disciplines to ensure patient care is seamless, efficient, and well-coordinated.
  • Be a Clinical Expert: Execute and meticulously document physician and nursing orders while constantly monitoring and communicating the patient's condition to the care team.
  • Holistic Assessment: Conduct thorough patient assessments and collaborate with the multidisciplinary healthcare team to proactively meet all of the patient's physical, emotional, and spiritual needs.

What minimum requirements you’ll need

Licensure / Certification / Registration:

Required Credential(s):

  • BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date. American Heart Association or American Red Cross accepted.
  • Registered Nurse credentialed from the Oklahoma Board of Nursing obtained prior to hire date or job transfer date.

Preferred Credential(s):

  • Advanced Life Support. American Heart Association or American Red Cross accepted.
  • Pediatric Advanced Life Support. American Heart Association or American Red Cross accepted.

Education:

  • Diploma from an accredited school/college of nursing OR Required professional licensure at time of hire.

What additional preferences we're seeking

  • Minimum 3 years RN ICU experience

MDS Coordinator

Maintain and periodically update written facility policies and procedures that govern the development, use and implementation of the Resident Assessment Instrument (RAI)/Minimum Data Set (MDS) and care plan. Develop, implement and maintain an ongoing quality assurance and performance improvement (QAPI) program for the resident assessment/care plans. Ensure that a current copy of the RAI Manual is available to persons completing portions of the MDS. Monitor the MDS website and portal for up-to-date changes in the RAI manual monthly; distribute changes in the RAI manual to the IDT as needed. Review quality measures reports monthly and make recommendations to the QAPI Committee. Complete electronic submission of required documentation to the state database and other entities in accordance with facility policies. Conduct and coordinate the completion and submission of MDS within the required timeframe. Submit and monitor the nursing home final validation report to verify assessment submission. Transmit MDS to the Centers for Medicare and Medicaid Services (CMS) information system for each resident contained in the MDS in a format that conforms to current formatting standards within the prescribed time frames.
