Overview

Geriatric Advance Practice Provider role in value-based care model. Monday-Friday, no weekends or evenings schedule. Salaried role with strong annual quality-based bonus. Patient panel across Denton, Aubrey, Lewisville, Carrollton areas with 30-45 min patient visits seeing approx. 6-9 patients per day.

Responsibilities

The Nurse Practitioners we seek are those who have directly related experience in home health, geriatrics, hospice and primary care. The Nurse Practitioner who wants to practice medicine with a focus on patient care, not volume. We want our clinicians to take the time needed to truly address the patient’s needs.
