We are seeking a skilled and compassionate Licensed Practical Nurse to join our dedicated team at Emerald Nursing & Rehab Legacy Pointe. As an LPN, you will play a vital role in providing high-quality resident care to our residents, working collaboratively with our interdisciplinary team to achieve individualized care plans. Responsibilities:

  • Provide high-quality resident care to residents, including administering medications, wound care, and other treatments as directed by the medical staff.

  • Perform vital signs, monitor patient conditions, and report any changes or concerns to the nursing team.

  • Develop and implement individualized care plans, collaborating with the interdisciplinary team to ensure resident needs are met.

  • Maintain accurate and detailed records of resident care, treatments, and progress.

  • Participate in quality improvement initiatives to enhance patient outcomes and satisfaction.

  • Foster a positive and respectful environment, promoting dignity and comfort for our residents.

Requirements:

  • Current licensure as a Licensed Practical Nurse (LPN) in the state of Nebraska.

  • High school diploma or equivalent required.

  • Basic Life Support (BLS) certification.

  • Ability to effectively communicate with residents, families, and healthcare team members.

  • Strong interpersonal and teamwork skills, with a focus on providing excellent patient care.
