Job Responsibilities

Providing hands-on nursing care to residents in post-acute care settings
Adhering to individualized resident care plans
Reviewing and maintaining accurate resident charts and medical records
Monitoring residents' physical and mental health and keeping a close watch on vital signs
Verifying, administering, and tracking the effects of resident medications
Assisting with IV setup, phlebotomy procedures, and immunizations
Performing resident intakes and thoroughly documenting medical histories and health concerns
Contributing to a safe, clean, and comfortable facility environment

Qualifications and Skills

Applicable state-issued LPN license
Successful completion of approved LPN program
Minimum of 6 months of experience (LPN or comparable role)
Ability to lift a minimum of 50 lbs
