We are seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our team at Constant Care Health Services LLC, a leading provider of skilled nursing facility services in The Bronx, New York. As an LPN at our Skilled Nursing Facility, you will play a vital role in delivering high-quality patient care, promoting independence, and enhancing the overall well-being of our residents.

Responsibilities

  • Provide direct patient care to residents in our Skilled Nursing Facility, including administering medications, wound care, and other treatments as prescribed by physicians

  • Monitor and report changes in patients' conditions to the nursing team, physicians, and family members

  • Collaborate with other healthcare professionals to develop and implement individualized care plans

  • Educate patients and their families on self-care techniques, medication management, and disease prevention

  • Participate in ongoing quality improvement initiatives to ensure the highest level of patient care
