Position Summary Utilizing the Nursing Process of assessing, planning, implementing and evaluating, the Registered Nurse is responsible for delivery of patient care and promoting collaboration among the multi disciplinary health care team.
Role Description The Health Promotion & Screening Staff Nurse is a full-time, on-site role based in Savannah, GA. This nurse provides community-focused health promotion, including conducting screenings, delivering health education, and supporting preventive care initiatives across diverse populations. Day-to-day responsibilities include assessing individual and community health needs, coordinating and implementing outreach events, and collaborating with interdisciplinary teams to improve health outcomes. The nurse documents assessments and interventions, maintains accurate records, and ensures compliance with organizational policies and clinical guidelines. The role also involves building trusting relationships with community members, connecting individuals to appropriate resources, and supporting the broader mission of St. Joseph’s/Candler to advance wellness and disease prevention.
Qualifications
- Bachelor's of Science in Nursing - Required
- 1-2 Years Clinical - Required
- Two years Medical/Surgical and/or Critical Care in a hospital within the last five years preferred.
- Demonstrated skills in health promotion and community health to support preventive care and outreach programs.
- Strong communication and health education abilities to effectively engage, counsel, and teach individuals and groups.
- Ability to work collaboratively with interdisciplinary teams and community partners, and to function independently in field settings.
- Solid clinical assessment, documentation, and organizational skills, with proficiency in basic electronic health record (EHR) use.
- Commitment to equitable, culturally sensitive care and alignment with the mission and values of a faith-based, not-for-profit health system.

- Registered Nurse License with State of Practice - Required
- Basic Life Support (BLS) - Required
- Advanced Cardiovascular Life Support (ACLS) - Required
- Core Job Functions
- Open communication with patients/families is utilized to achieve positive patient/family outcomes. Follow up on patient/family/customer complaints in a timely manner. Attends community activities including health fairs, lectures, and visits to doctor offices.
- Collects and reviews information on lifestyle. Educates based on health risks and needs, sets realistic goals and reviews as needed. Researches materials to stay current on treatments of chronic disease.
- Follow the provider orders for blood draws, urinalysis, and other CLIA waived tests. Prepare labs for courier pick up. Ensure test tubes, specimens labeled accurately.
- Identifies opportunities for community education that support the strategic plan. Assists with coordinator of speaking and educational opportunities.
