WHO WE ARE LOOKING FOR:

Franciscan Physician Network is seeking a Cardiology Nurse Practitioner to join our group at Franciscan Physician Network Cardiology Crown Point located in Crown Point, Indiana. Interested candidate will join an extensive and established cardiology team.

WHAT WE NEED:

Ideal candidate will have past or present IMCU, CVU and inpatient cardiology experience, strong focus on clinical excellence, and patient satisfaction. Candidate will display exceptional interpersonal skills and desire to develop strong ties with the community and existing medical staff. Provider will need to work collaboratively with physician to coordinate and deliver the highest quality of patient care.

WHAT YOU CAN EXPECT:

  • Schedule: 36 Clinical Patient Contact Hours

  • Average inpatient volume: 15 - 20 patients per day; inpatient rounding shared on weekends call 1:4 to 1:5

  • Outpatient volume will vary

  • IMCU, CVU & inpatient cardiology experience is preferred

  • Candidate will manage:

    • H&P’s

    • Discharge orders

    • Discharge summaries

As an Advanced Practice Provider, you are responsible for comprehensive direct patient care management. In this role, you will utilize advanced assessment skills and knowledge of clinical therapeutics to evaluate, diagnose and manage patients on the Hospitalist Surgical Co-management Service. The APP orders and interprets appropriate diagnostic tests, therapeutic interventions and prescribes pharmacological agents as necessary. As an APP, you will maintain continuity of patient care through patient follow-up, monitoring of patient progress and modifying plan of care as necessary and per scheduling requirements. The APP applies knowledge of new technology and resources as per competencies obtained through the mentorship/proctoring of the surgical co-management attendings demonstrating advanced or specialized expertise. This role will be a part of a team of Advanced Practice Providers working in the Hospitalist Surgical Co-management space. This role will train with our team at the Hyde Park campus with involvement in onboarding with the following specialties Thoracic, Orthopedics, General Surgery, Urology, and Vascular.
