The Psychiatric Nurse Practitioner is responsible for providing comprehensive psychiatric services to incarcerated individuals within a correctional facility and with a collaborating physician. This includes assessing, diagnosing, and treating psychiatric disorders and collaborating with a multidisciplinary team to ensure the delivery of quality healthcare services within a medical home model.

The successful applicant should be able to perform all of the following functions at a pace and level of performance consistent with the actual job performance requirements:

Psychiatric Assessment and Diagnosis:

  •     Conduct thorough psychiatric evaluations and assessments of incarcerated individuals to determine the presence and severity of mental health disorders.
  •      Diagnose and classify mental illnesses according to established diagnostic criteria (e.g., DSM-5).
  •     Assess and evaluate suicide risk, self-harm, and dangerous behaviors among incarcerated individuals.
  •      Collaborate with other healthcare professionals and correctional staff to gather relevant information for accurate diagnoses.

Treatment Planning and Implementation:

  •     Develop and implement individualized treatment plans for incarcerated individuals with psychiatric disorders.
  •     Prescribe appropriate psychiatric medications and monitor their effectiveness and side effects.
  •      Provide brief psychotherapy and counseling services to incarcerated individuals, using evidence-based therapeutic modalities.
  •     Collaborate with the interdisciplinary team to coordinate care and ensure a holistic approach to treatment.
  •     Modify treatment plans as needed based on patients' progress and response to interventions.

Crisis Intervention and Emergency Services:

  •    Respond to psychiatric emergencies within the correctional facility promptly and efficiently.
  •     Conduct crisis assessments, provide stabilization, and implement appropriate intervention strategies.
  •     Collaborate with correctional staff and healthcare team members to implement emergency response protocols.
  •    Determine the need for a higher level of psychiatric treatment when necessary.

Collaboration and Consultation:

  •      Collaborate with other healthcare professionals, including primary care physicians, psychologists, social workers, and nurses, to provide integrated care to incarcerated individuals.
  •     Consult with the interdisciplinary team to develop comprehensive treatment plans for incarcerated individuals with complex psychiatric needs.
  •     Provide expertise and guidance to correctional staff regarding mental health issues, crisis management, and suicide prevention.
  •     Participate in case conferences, treatment team meetings, and quality improvement initiatives.

Documentation and Compliance:

  •     Maintain accurate and up-to-date psychiatric records for each incarcerated individual, including assessments, diagnoses, treatment plans, and progress notes.
  •     Ensure compliance with relevant laws, regulations, and ethical guidelines pertaining to correctional healthcare and patient confidentiality.
  •      Adhere to the correctional facility's policies and procedures related to documentation and record-keeping.

Education and Training:

  •     Stay updated with the latest advancements in psychiatric medicine, treatment modalities, and relevant research.
  •      Engage in continuous professional development and relevant educational activities.
  •    Provide training and education to correctional staff on mental health topics, crisis management, and de-escalation techniques

Qualifications: 

  • Board certification as a Psychiatric Mental Health Nurse Practitioner
  • Current and unrestricted Nurse practitioner license in the state/jurisdiction where the correctional facility is located.
  • Experience in providing psychiatric care or relevant experience in correctional or underserved populations is preferred.
  • Strong assessment and diagnostic skills, with the ability to manage a wide range of acute and chronic medical conditions.
  • Proficiency in prescribing medications and therapies under physician supervision
  • A valid driver's license is required.
  • Minimum age requirement: Must be at least 18 years of age.

How you make a difference The Nurse Practitioner (NP) under the supervision of the Site Medical Director and other site Physicians, offers a comprehensive range of medical services to incarcerated patients in accordance with established policy and state guidelines. They ensure necessary documentation of services is provided to the Site Medical Director or designee to monitor the provision of clinical services. Additionally, they communicate scheduling changes to the Medical Director and H.S.A. and make arrangements for coverage of medical services if unavailable for an extended period of time. Job Description Summary The Nurse Practitioner is a registered professional nurse with advanced graduate education and clinical training who provides comprehensive hospice services in compliance with the state Nurse Practice Act and under the supervision of a collaborating physician. He/She provides hospice care through the evaluation and assessment of the health history, health promotion, diagnosing medical conditions, management of health problems by directing and developing the plan of care, prescribing medications or treatments, coordinating and collaborating with patients, families and other healthcare practitioners. Under the supervision of a physician, the nurse practitioner visits patients, completes physical assessments, performs face to face assessments and evaluates hospice eligibility as assigned in consultation with the hospice interdisciplinary team. The nurse practitioner builds from the resources of the community to plan and direct services to meet the need of individuals and families within their homes and communities. Essential Job Functions/Responsibilities Patient Care 1. Provides medical care, palliates and manages terminal illness in compliance with the state Nurse Practice Act under the supervision of a collaborating physician. 2. Completes history and physical examinations by evaluating the patient's medical condition and health history, ordering and interpreting results from laboratory and diagnostic tests or procedures, diagnosing health conditions and documents the findings in the patient's medical record. 3. Manages health conditions and terminal illnesses by developing an individualized plan of care, prescribing medications and / or treatments, obtaining consultations or making referrals and coordinating hospice care services. 4. Completes the hospice face to face visit requirements prior to the third and subsequent hospice benefit periods to assess hospice eligibility based on the hospice conditions of participation and the local coverage determinations. Provides clinical information to the hospice medical director for interpretation for eligibility for hospice recertification. 5. Makes recommendations to the hospice medical director regarding patient eligibility for certification and recertification for hospice care. 6. Reviews clinical information, coordinates and oversees patient care in consultation with the hospice interdisciplinary team and other healthcare practitioners. 7. Assures that patient receives appropriate measures to control symptoms, through collaboration with interdisciplinary team members. 1. Reviews and develops protocols for treatment and proposes options for interventions based on the literature in collaboration with the collaborating physician. 2. Consults the collaborating physician when the patient's plan of care is outside standardized practice and protocols. Communication 1. Communicates and collaborates with the interdisciplinary group to create, review and revise the patient's plan of care. 2. Consults with the patient's attending physician, the hospice medical director and other healthcare practitioners regarding the patient's eligibility for hospice care, ongoing care needs and medical management related to the patient's terminal condition. 3. Communicates with other community health practitioners to coordinate the plan of care. 4. Attends and participates in hospice interdisciplinary group meetings. 5. Completes, maintains and submits accurate and relevant clinical notes, physical examination notes, assessment visit notes, medical orders, collaboration notes and other documentation in the medical record. 6. Educates/counsels patients, families, and/or caregivers as to preventative care, medical problems, psychological problems, and spiritual problems in conjunction with the interdisciplinary team to meet the total needs of patients. 7. Provides and maintains a safe environment for the patient. 8. Assists the patient and family/caregiver and other team members in providing continuity of care. 9. Works in cooperation with the family/caregiver and hospice interdisciplinary group to identify the goals of care and meet the care needs of the patient and family/caregiver. 1 Serves as a nursing resource for consultation and educations to members of the interdisciplinary team and other healthcare practitioners in the community. 1 Establishes, builds and nurtures relationships with staff and community referral sources to facilitate program growth. Additional Duties 1. Maintains knowledge of and compliance with current Medicare/Medicaid, state/federal rules and regulations for hospice services 2.
