About the Opportunity:

Gastro Health is seeking an experienced Advanced Practice Provider (NP) to join our private practice team in Frederick, Maryland. Our APPs play an active part in delivering high-quality GI care by coordinating individualized treatment plans under physician supervision and engaging in clinical responsibilities that help drive high-quality, patient-centered care.

  • Work collaboratively with a team of 3 gastroenterologists, 2 APPs, and support staff
  • Monday through Friday schedule: 8:30AM to 5PM

    Nurse Practitioner – Wound & Primary Care at Home | $15K Sign-On

    Overview Meritus Health is building a high-autonomy house-calls program for patients who need wound care and longitudinal primary care in their homes and in our Home Health clinic. We’re looking for a Nurse Practitioner who wants to own their panel, practice at the top of license, and be the clinical backbone of this model. Role Summary In this role, you will provide in-home visits for wound care and primary care, with the potential for some clinic-based sessions at our Home Health department location. You will manage a defined panel of patients, coordinate with Home Health nurses and physicians, and have significant day-to-day independence in how you structure your schedule and care plans. Key Responsibilities Provide comprehensive in-home primary care: history, physical exam, diagnosis, and ongoing management of chronic and acute conditions. Deliver wound care in the home and clinic, including assessment, debridement (within scope), dressing selection, and application of advanced therapies. Develop and own individualized care plans for your patient panel, with an emphasis on preventing avoidable ED visits and hospitalizations. Perform periodic clinic sessions at the Home Health location for follow-up, new consults, and procedures best done in a clinic setting. Coordinate closely with Home Health nurses, PT/OT, and referring physicians to ensure seamless transitions and aligned treatment plans.     Meritus Personal Health Institute (MPHI) About the Meritus Personal Health Institute Meritus Health is launching the Meritus Personal Health Institute (MPHI) — a first-of-its-kind population health and precision medicine initiative rooted in the rural communities of Western Maryland. Backed by a $24M initial investment , MPHI will enroll approximately 10,000 residents in a longitudinal program combining whole genome sequencing, advanced laboratory testing and imaging, pharmacogenomics, metabolic profiling, personalized health coaching, and comprehensive preventive health assessments — at no cost to the patient . The aspirational goal is to increase healthy life expectancy in rural Maryland by three years , while establishing Meritus Health as a national model for rural precision health. Position Overview The Nurse Practitioner will be an integral member of the MPHI clinical team, working closely with the Medical Director, APPs, health coaches, technicians, navigators, researchers, and physicians across Meritus Health. This role will provide advanced practice clinical care throughout the participant journey, including comprehensive health assessments, interpretation and application of advanced diagnostic information, preventive health planning, risk-factor management, and longitudinal follow-up. The NP will help translate the program's precision-health data into practical, personalized recommendations that participants and their primary care teams can use to improve health and prevent disease .
