Job Summary:

Under physician’s orders and RN supervision, the Continuous Care (CC) Licensed Practical Nurse (LPN) is responsible for the observation of patient/family needs and providing supportive care and emotional support to the patient/family unit in times of crisis in order to maximize patient comfort and enhance quality of life.

Responsibilities:

1. Under the direction of the RN case manager, provides continuous bed-side clinical observations, prescribed care, physical, and emotional support to patients as directed in plan of care, reporting significant changes in patient condition and family needs as needed.

2. In collaboration with the RN, accepts and processes physician orders to maximize patient comfort.

Qualifications:

·         Graduate of an accredited practical nursing program.

·         Active Licensed Practical Nurse (LPN) in the State of New Jersey.

·         Minimum of 1-2 years of clinical experience, preferably in hospice, long-term care, home health, or acute care.

·         Understanding of Hospice philosophy and needs of terminally ill.

·         Strong problem solving and critical thinking skills.

·         Ability to travel within the state of NJ

This position will oversee the Evesham Township area. Responsibilities: Provide direct care at the patient’s place of residence as indicated. Work collaboratively as a member of the Interdisciplinary Team to assure that the aims of continuity and comprehensiveness of care are accomplished. Understand Hospice philosophy, principles of death / dying. Compassionate communication with their patients and their families. Provides professional nursing care by utilizing all elements of nursing process. Collaborates with the patient and family to promote the physical well-being and address coping issues that may arise.
