Qualifications

A heart to serve patients and families and a passion for providing the best possible care
Education: Graduate of an accredited masters, post masters, or doctoral nursing program with formal preparation as a Nurse Practitioner.
Licensure: Current unencumbered licensure as an Advanced Practice Registered Nurse with designation as a Nurse Practitioner. Current prescriptive authority with full DEA privileges. Must achieve and maintain national certification in the Nurse Practitioner specialty
Experience: 2 years experience within the last 5 years in chronic disease management/long-term care. Hospice and/or Palliative background preferred.
Required: Reliable transportation. Ability to sit, stand, bend, move intermittently and lift at least 80-100 lbs.

Create, document, and update individualized infusion care plans and maintain thorough, timely documentation in the EHR. Conduct initial patient evaluations for therapy appropriateness, including payer criteria and clinical suitability for infusion center setting. Maintain open, timely communication with medical directors, referring physicians, and interdisciplinary staff regarding care coordination. Identify urgent or emergent reactions during infusion, initiate appropriate interventions, and coordinate emergency transfers when necessary. Serve as a clinical leader within the infusion center team; provide guidance to infusion nurses and support staff on patient care and clinical protocols. Support patient education, counseling, and safe transition of care across healthcare settings. Participate in quality improvement initiatives and operational planning efforts across the infusion center. Participate in credentialing and contracting processes by maintaining all payer-required licenses and individual NPI enrollment linked to the organization’s Tax ID.
