Grand Itasca's LPN's/MA's work closely with providers to deliver quality patient care and service by maintaining patient flow, escorting patients through the clinic, identifying patient needs, collecting/verifying information, administering medications, updating electronic medical records, implementing methods as directed to improve care and the patient experience, maintaining exam/treatment rooms, ordering related supplies and display original thinking & creativity while working effectively as an important team member.

JOB SUMMARY:
Consistent with the Minnesota Nurse Practice Act and/or Commission of Accreditation of Allied Health Education Programs (CAAHEP) will provide patient care "through the understanding of nursing standards, recognized by the Minnesota State Board of Nursing. Care will be rendered under the direction of a registered nurse, advanced practice nurse, physician or physician assistant." When this job description applies to the assignment or acceptance of a float position, the responsibilities are equivalent; however the health care team and unit you are assigned to will direct specific job duties to meet the patient care needs.

 

JOB DUTIES:
1. The Licensed Practical Nurse conducts a focused assessment though the collection and comparison of data to normal findings and the patient's current health status. Report of changes will be given to the appropriate licensed health care provider for on-going direction, patient management and delegation of care.

2. The Licensed Practical Nurse participates with health care team in the development and modifications to the patient's health plan of care plan/orders.
3. The Licensed Practical Nurse implements patient care interventions that are delegated, ordered or prescribed by a licensed health care provider.
4. The Licensed Practical Nurse assigns nursing activities or tasks to other licensed practical nurses (LPN).
5. The Licensed Practical Nurse assigns and monitors nursing task or activities that are delegated to certified medical assistants or any unlicensed personnel on the health care team.
6. Focus on patient care through the provision of safe /effective care, patient advocacy, assistance with patient evaluation to interventions and collaboration and communication with health care team members.
7. Provides health care information to patients and reviews education materials.
8. Provides input for policies and procedures
9. Consistent with section 148.211, subdivision 1 of the Minnesota Nurse Practice Act "Accountable for the quality of care delivered, recognizes the limits of practice and competency, and preforms to the level of education knowledge and skill ordinarily expected of individuals who have competed an approved LPN education program."

 

QUALIFICATIONS:

1. Current LPN Minnesota state license, current Medical Assistant certification or registration required. Must maintain license/certification/registration.
2. Experience in an ambulatory health care setting is preferred.
3. Ability to work efficiently, accurately, and maintain positive working relationships.
4. BLS certification to be completed within two months of employment.
5. Demonstrated ability to follow and adhere to Grand Itasca Clinic and Hospital policies, vision, mission and values.
6. Maintains reliable and consistent attendance.

Offsite Medical Assistant I Ambulatory-Obstetrics And Gynecology Administration-Mount Sinai West - Full Time Day

Description The Medical Assistant I provides clinical office support to physicians and surgeons and performs patient care and administrative duties necessary to deliver quality patient care. This individual is also responsible for assisting with clinical, administrative and organizational tasks associated with patient care coordination. Responsibilities 1. Assists the provider during patient visit via obtaining vital signs (height, weight, blood pressure, pain scale etc) and documenting vital sign results and transcribing medication information into the electronic medical record (EMR). 2. Obtains prior authorizations for prescriptions and durable medical equipment 3. Drafts prescription requests in EMR and sends to provider for review, completion and processing 4. Performs Phlebotomy services inclusive of collecting and processing blood, urine and stool specimens and use of a centrifuge for spinning bloods 5. Performs POCT (w/the exception of glucose) and documents results in EMR for provider to review 6. Performs daily Quality Control for all POCT equipment 7. Orders clinical supplies when necessary 8. Maintains and stocks exam and consult rooms with clinical supplies and clerical forms used by Providers 9. Assists providers with procedures by preparing supplies and equipment and acting as a chaperone as needed. 10. Perform EKGs when ordered by provider 11. Performs pre-visit planning for Diabetic and CHF patients as per practice guidelines 12. Monitor and maintains refrigerator temperature logs and biomed expirations for medical equipment 13. Participates in daily huddles with MDs and other clinical team members to review daily care management patient lists and discuss patient's treatment plan & management 14. Performs age-specific screenings for Activities of Daily Living, Falls, Alcohol, Tobacco, and/or Depression screenings annually for every patient 15. Enters data into the electronic medical record related to medication adherence, diabetes control, foot/eye exam compliance for provider review and reconciliation 16. Assists providers with distributing educational materials to patients 17. Assists Population Health Management team by verifying patient has completed their colorectal screening, breast screening, cervical cancer screening and other pertinent preventive screens. 18. Retrieves information regarding hospital admission and Emergency Room visits from outside facilities for evaluation by provider for transition of care coordination 19. Assists providers with follow-up related to patients on Coumadin therapy 20. Assists at the front desk as needed: answer telephones, register patients, discharge patients, schedule appointments, copy charts, schedule ancillary tests, and follow up on overdue diagnostic test results. 21. Follows Primary Care Medical Home and MACRA protocols for designated role 22. Performs all other job related functions as assigned.
