Key Responsibilities

  • Build meaningful and supportive relationships with individuals receiving behavioral health services.
  • Provide nursing assessments, care coordination, health education, and clinical support that promote health, recovery, and overall wellness.
  • Support medication management, medication education, and treatment adherence while helping individuals achieve their goals.
  • Collaborate with interdisciplinary team members, healthcare providers, families, and community resources to ensure coordinated care.
  • Monitor health concerns, treatment progress, and emerging needs while supporting timely intervention and follow-up care.
  • Educate individuals on medications, health conditions, wellness practices, prevention strategies, and self-management skills.
  • Coordinate referrals, communication, and continuity of care among internal and external providers.
  • Advocate for individuals by helping reduce barriers to treatment, healthcare access, and support services.
  • Maintain accurate clinical documentation in accordance with organizational and regulatory standards.
  • Participate in treatment planning, interdisciplinary team meetings, quality improvement initiatives, and other assigned responsibilities.
     

Education, Experience, and/or Credential Qualifications

  • Hold a current Registered Nurse (RN) or Licensed Practical Nurse (LPN) license in good standing.
  • Demonstrate knowledge of nursing practices, health assessment, medication administration, and person-centered care principles.
  • Possess strong communication, clinical decision-making, and critical-thinking skills.
  • Bring experience in behavioral health, mental health, substance use treatment, addiction recovery, residential services, community health, outpatient services, case management, or related healthcare settings preferred.
  • Maintain all applicable licensure, certifications, and professional standards required for nursing practice.
     

Additional Qualifications

  • Maintain a valid driver's license, acceptable driving record, and current auto insurance.
  • Demonstrate strong organizational, communication, and relationship-building skills.
  • Work effectively with individuals experiencing behavioral health, mental health, substance use, and complex healthcare needs.
  • Collaborate successfully within a multidisciplinary healthcare and behavioral health team.
  • Manage multiple priorities while maintaining professionalism and attention to detail.
  • Promote recovery-oriented, trauma-informed, and person-centered care practices.
     

Physical Requirements

  • Perform sedentary to light work involving documentation, computer use, communication, and client interaction.
  • Travel as needed to support client care, service coordination, and program operations.
  • Move between office, clinic, residential, and community-based settings while performing essential job functions.
  • Lift, carry, push, or pull up to 10 pounds occasionally.
  • Maintain the ability to perform essential job duties with or without reasonable accommodation.
