Job Summary

As a Registered Nurse MDS Coordinator (RN) (MDS), you will play a vital role in ensuring that our residents receive the highest level of care and attention. Responsibilities

  • Coordinate and conduct Minimum Data Set (MDS) assessments for residents, ensuring accurate and timely documentation

  • Develop and implement individualized care plans, working closely with the healthcare team to achieve resident goals

  • Monitor and track resident progress, identifying areas for improvement and implementing necessary changes

  • Collaborate with physicians, therapists, and other healthcare professionals to ensure comprehensive care

  • Maintain accurate and up-to-date records, adhering to regulatory guidelines and company policies

  • Provide education and training to staff members, promoting best practices and continuous learning

  • Participate in quality improvement initiatives, contributing to the development of policies and procedures

Requirements

  • Registered Nurse (RN) license with current registration in the state of Nevada

  • Strong clinical skills and experience in long-term care, preferably in SNF settings

  • Ability to work effectively in a fast-paced environment, prioritizing multiple tasks and responsibilities

  • Excellent communication and interpersonal skills, with the ability to work collaboratively with residents, families, and healthcare team members

  • Ability to maintain confidentiality and adhere to HIPAA guidelines
