As a Licensed Practical Nurse (LPN) on our team, you will play a vital role in delivering high-quality patient care to our residents. We are seeking a compassionate and skilled nurse to provide medical and rehabilitation services to our short-term and long-term care residents.

Licensed Practical Nurse (LPN) Responsibilities:

• Provide direct patient care to residents, including administering medications, wound care, and vital sign monitoring
• Develop and implement individualized care plans in collaboration with the healthcare team
• Document resident progress, report any changes or concerns to the healthcare team, and participate in interdisciplinary care conferences
• Collaborate with other healthcare professionals to ensure seamless transitions and optimal outcomes
• Maintain accurate and up-to-date medical records

Licensed Practical Nurse (LPN) Requirements:

• Current LPN licensure
• 0-2 years of experience in a post-acute care setting (new graduates welcome to apply)
• Strong communication and interpersonal skills
• Ability to work collaboratively as part of a team
• Basic computer skills and familiarity with electronic medical records
