What You’ll Do
As a Hospice LPN, you will provide skilled, compassionate nursing care under the direction of an RN while helping patients remain comfortable and maintain their dignity and quality of life.
Your day may include:
Providing individualized, patient-centered care based on each patient’s care plan
Administering medications and treatments as ordered
Monitoring patients and communicating changes to the RN and care team
Helping patients and families understand care needs, medications, and comfort measures
Maintaining accurate and timely clinical documentation
Collaborating with the interdisciplinary hospice team
Building trusting relationships with patients, families, and caregivers
Being a compassionate and dependable presence when patients and families need it most
What We’re Looking For
Active Pennsylvania LPN license required
Previous hospice or home health experience preferred
Strong clinical and assessment skills
Excellent communication and interpersonal skills
Compassionate, patient-focused approach
Ability to work independently while being an engaged member of a team
Valid driver’s license and reliable transportation
