We are seeking a highly skilled and compassionate LPN Licensed Practical Nurse Per Diem to join our team!

LPN Licensed Practical Nurse Responsibilities:

  • Provide direct patient care, under the supervision of a registered nurse, to residents with a variety of needs and conditions

  • Administer medications and treatments as prescribed by the physician

  • Monitor and record vital signs, and report any changes to the registered nurse

  • Develop and implement individualized care plans to meet the unique needs of each resident

  • Collaborate with the interdisciplinary care team to ensure comprehensive care

  • Participate in quality improvement initiatives to ensure the highest level of care

LPN Licensed Practical Nurse Requirements:

  • Current LPN license in the state of Pennsylvania

  • Prior experience in a long-term care or rehabilitation setting preferred
