LVN Hospice Full-time

Job Summary:

Primary function is to administer skilled nursing care, under the supervision of a registered nurse, for clients of all ages in their place of residence, coordinate care with the interdisciplinary team, patient/family, and referring agency.

Lines of Authority: Reports to the Director of Nursing

Job Qualifications:

Education:        Graduate of an accredited school of vocational nursing

Licensure:        Current state license as a Licensed Vocational Nurse, current state Driver’s License

Experience:      Two years’ experience as a Licensed Vocational Nurse in a clinical care setting, hospice, or home health preferred.

Skills:               Nursing skills as defined as generally accepted standards of practice. Good interpersonal skills. Proof of Hepatitis acceptance or declination.

Transportation:    Reliable transportation. Valid and current auto liability insurance.

Environmental and Working Conditions:

Works in patient’s home in various conditions, possible exposure to blood and body fluids and infectious diseases; ability to work flexible schedule, ability to travel locally; some exposure to unpleasant weather, PRN emergency call

Physical and Mental Effort:

Prolonged standing and walking required. Ability to move up to 50 lbs. and move patients. Requires working under some stressful conditions to meet deadlines, to identify patient needs, to make quick decisions and resource acquisition; meet patient/family psychosocial needs. Requires hand-eye coordination and manual dexterity. Ability to utilize durable medical equipment in the home.

 

Essential Functions:                                                                                                                                                        

Under the direction of the RN, supplements the nursing care needs of the patient as provided by the RN and as specified in the POC.

Assist the RN in carrying out the plan of care.

Assist the patient in learning appropriate self- care techniques.

Complete appropriate clinical and progress notes, documenting outcomes of interventions.

Provide services in accordance with organizational policies and procedures and regulations that define scope of practice.

Assist the RN and/or physician in performing specialized duties related to end-of-life care.

Prepare equipment and materials for treatment adhering to aseptic technique as required.

Assess patient/family response to care, ensuring communication of information to appropriate team members.

Promote Agency philosophy and administrative policies to ensure quality of care.

JOB DESCRIPTION Opportunity for a Texas licensed LVN to join Molina as a Field Care Manager to work with our Medicare members in the service delivery area in the northwestern area of Houston, covering communities that include Cypress, Tomball, and Jersey City. You will complete assessments needed for determining the types of services our members are eligible to receive. Preference will be given to those candidates with previous experience working with the Medicare population within a Managed Care Organization (MCO). Mileage is reimbursed as part of our benefits package, but we are only considering candidates who are within 30 – 45 minutes of the coverage area. Hours are Monday – Friday, 8 AM – 5 PM CST. Solid experience with Microsoft Office Suite is necessary, especially with Outlook, Excel, One Note and Teams as well as being confident in toggling between different programs to complete the necessary forms and documentation. Job Summary Provides support for care management/care coordination long-term services and supports (LTSS)-specific activities. Collaborates with multidisciplinary team coordinating integrated delivery of member care across the continuum for members with high-need potential. Required Qualifications • At least 2 years of health care experience, including at least 1 year of experience working with persons with disabilities/chronic conditions long-term services and supports (LTSS), and 1 year of experience in care management, or experience in a medical and/or behavioral health setting, or equivalent combination of relevant education and experience. • Clinical licensure and/or certification required ONLY if required by state contract, regulation, business operating model, or state board licensing mandates.
