Compensation: Pay per visit, $45 - $115 per visit (amount dependent upon visit type), $270 - $320+ per day, Mileage reimbursed at $0.55/mile, and Payroll processed bi-weekly (26 pay periods).

Summary:

We are looking for compassionate Registered Nurses (RNs) to work one-on-one with our patients in their home environment to establish and oversee all aspects of case-managing patient care. Case management involves creating a Plan of Care for patients and following physician orders that lead to outcomes of increased patient independence and satisfaction. The RN for this position enjoys working with a team of supportive clinicians as well as being disciplined to ensure the highest level of care.

Primary Responsibilities:

  • Assumes responsibility for assigned patient’s care from admission through discharge.

    • Includes continued monitoring, reassessment, and re-evaluation throughout the episode.

  • Understands the patient’s reimbursement coverage and requirements for ongoing care eligibility.

    • Monitors and ensures visits are authorized and provides needed information for ongoing authorization.

  • Assesses patient and caregiver needs at OASIS time points and throughout the episode.

  • Develops, communicates, and coordinates the Plan of Care with other disciplines in collaboration with the ordering physician, including the coordination of visit frequency.

  • Provides patient and caregiver education including, but not limited to, protocols and disease management programs.

  • Manage internal and community resources needed to implement the Plan of Care.

  • Plans and leads weekly care team update meetings.

  • Report on patient progress to clinical management on a regular basis.

  • Collaborates with patient, family / caregiver(s), MD, other care team members, and community resources.

  • Evaluates and follows up on all pertinent aspects of care (lab work, medication changes, sudden changes in status, etc.).

  • Participates in discharge planning / recertification decision in collaboration with other care team members.

  • Makes changes to the care plan as needed and initiates verbal orders and / or collaborates with other visit clinicians.

  • Oversee and supervise the work of Licensed Practical Nurses and other clinicians.

    • LPN Supervisory Visits are performed roughly every 30 days.

  • Submit accurate, complete, timely, meaningful and required documentation, including progress and clinical notes.

  • Participate in case conferences with other clinicians by reviewing: services, clinical documentation and discussing pertinent patient care issues and goals.

  • Participate in on-call rotation as reasonably assigned.

Qualifications:

  • Preference for 2+ years in an acute or a post-acute care setting.

  • Active State Registered Nurse licensure is required.

  • Proof of current CPR certification and valid Driver’s license.

  • Proof of current auto insurance coverage.
