Responsibilities for Licensed Practical Nurse (LPN):

  • Provide patient care in accordance with accepted standards of nursing practice, the written plan of care, and the physician’s plan of treatment.

  • Follow and implement individualized care plans developed by the Nurse Manager and interdisciplinary team.

  • Perform medication administration and prescribed treatments in accordance with the plan of care (POC) and facility policies.

  • Monitor residents’ conditions, report changes promptly to the supervising nurse or physician, and respond appropriately to clinical needs.

  • Maintain accurate, timely documentation in accordance with physician orders, care plans, and regulatory standards.

  • Support a safe, respectful, and therapeutic environment for residents and their families.

Qualifications for Licensed Practical Nurse (LPN):

  • Current, unencumbered active LPN license in the state of Pennsylvania.

  • Valid BLS Certification (CPR) required.

  • Ability to care for and manage multiple patients in a fast-paced healthcare environment.

  • Strong clinical judgment, communication skills, and attention to detail.

A Brief Overview Children's Hospital of Philadelphia is seeking two Licensed Practical Nurses for their Highpoint Primary Care office. This position will be Monday-Friday and will require one evening shift per week and a rotating Saturday shift. The shift will be based off of the operational need. Two years of LPN and pediatric experience is preferred. The Licensed Practical Nurse performs routine nursing care of the ill, injured or infirm under the direction of a licensed professional (RN) nurse, nurse practitioner and/or a physician. The LPN participates in the planning, implementation and evaluation of nursing care in accordance with the scope of the LPN as defined by the State Board of Nursing and health system standards. Also supports operations through administrative duties as required. The nurse must demonstrate competence in caring for patients with moderate to complex health issues and assist the Site Manager with administrative issues which affect the clinical practices within the office as well as comply with the administrative policies and clinical protocols as outlined in policy and procedure manual. Administratively responsible to the Site Manager and Ambulatory Nursing Director for nursing practice issues. Must be willing to rotate if needed to other Care Network sites within the designated cluster. What you will do Provide and document direct nursing care of assigned patients under the supervision of a registered nurse or physician. Nursing care is guided by physician orders and the plan of care. Patient response to care is reported to a registered nurse or physician for evaluation, intervention and modification of the plan of care.
