As a Field RN, you'll travel throughout Schenectady County to complete patient assessments, oversee individualized Plans of Care, supervise home health aides, and help ensure clients receive safe, consistent, and high-quality care.

What You'll Do

  • Travel to patient homes and community-based settings throughout Schenectady County

  • Conduct comprehensive in-home nursing assessments

  • Perform head-to-toe evaluations, including medication and diagnosis reviews

  • Develop, review, and update individualized Plans of Care (POC)

  • Complete required home health aide supervisions

  • Provide clinical guidance and support to aides and caregivers

  • Orient aides to each patient's Plan of Care and individual care needs

  • Monitor patients for changes in condition and communicate concerns appropriately

  • Collaborate with patients, families, caregivers, aides, and office staff

  • Complete accurate and timely clinical documentation

  • Help maintain continuity, safety, and quality of patient care

  • Maintain compliance with applicable healthcare regulations and company policies

What We're Looking For

  • Current and active New York State Registered Nurse (RN) license

  • Ability and willingness to travel throughout Schenectady County and assigned service areas
