Full Time Registered Nurse Case Manager, Level 2 Anticipated Territory: Sebastopol and surrounding areas What you’ll do as a Registered Nurse (RN), Level 2 Home Health Case Manager: Utilize your outstanding clinical skills as you create, implement and execute the overall plan of care to promote wellness and prevent re-hospitalization for patients in their homes. In this leadership role, you will have oversight of 2 Nurses who are responsible for executing your care plan. You will be responsible for conducting comprehensive patient assessments such as Start of Cares (SOCs), Evaluations, Resumption of Care and discharge planning.
