In this role, you will perform in-home wellness visits and risk adjustment assessments for Medicare and other eligible patient populations. Your work will contribute to accurate clinical documentation, identification of care gaps, and delivery of high-quality, value-based healthcare. Primary Purpose The State Traveler Provider is responsible for conducting comprehensive in-home health assessments, Annual Wellness Visits, and preventive screenings while traveling throughout Southern Michigan. Key Responsibilities In-Home Clinical Assessments Travel to members' homes and conduct comprehensive health evaluations.
