Job Summary

We are seeking experienced RN or LPN Nurse Managers to join our team at Burlington Health & Rehabilitation Center in Burlington, Vermont. As a Nurse Manager, you will provide direct nursing care to residents and supervise the day-to-day nursing activities performed by licensed practical nurses and certified nursing assistants. Responsibilities

Direct the daily activities of licensed practical nurses and certified nursing assistants in accordance with current federal, state, and local regulations, guidelines, and facility policies
Follow the nursing model to promote holistic, resident-centered care
Ensure nursing personnel comply with facility policies and procedures
Participate in resident admissions, discharges, and transfers
Assess residents for changes in condition, notify physicians and families or representatives, and document accordingly
Transcribe physician orders and carry out orders as written
Prepare and administer medications as ordered and monitor for adverse effects
Perform wound treatments as ordered and document changes appropriately
Schedule follow-up appointments and coordinate transportation needs
Provide nursing leadership and guidance to nursing personnel assigned to the unit
Provide direct care, including colostomy changes, tube feedings, wound care, suctioning, IV administration, and other skilled nursing services as assigned
Conduct rounds to ensure resident needs are being met and staff are performing their assigned duties
Collaborate with interdisciplinary team members to ensure residents' needs are fully addressed
Participate in management team meetings to discuss census changes, resident status, care needs, complaints, and concerns
Assist with evaluating nursing staff performance and implement disciplinary action according to facility policies and direction

Qualifications

Current, unrestricted Vermont RN or LPN license
Nursing degree from an accredited college or university
Current CPR certification
Prior long-term care experience preferred but not required
Strong leadership, communication, and critical-thinking skills
Ability to work effectively in a fast-paced healthcare environment
