Job Summary:

The Geneva Center for Rehabilitation and Nursing is seeking a compassionate and skilled Evening LPN to join our dynamic healthcare team. As a Evening LPN, you will play a vital role in providing high-quality patient care and support to our residents. In this role, you will work closely with our interdisciplinary team to deliver exceptional care, promote independence, and enhance the overall well-being of our patients.

Responsibilities:

  • Provide direct patient care under the supervision of registered nurses and physicians, including administering medications, performing vital signs, and assisting with daily living activities

  • Develop and implement individualized care plans to meet the unique needs of each patient

  • Monitor and report patient progress, identifying areas for improvement and implementing changes as necessary

  • Collaborate with the interdisciplinary team to ensure comprehensive care and coordination of services

  • Maintain accurate and up-to-date records of patient care and treatment

  • Participate in quality improvement initiatives to enhance patient outcomes and satisfaction

  • Provide education and support to patients and their families on health-related topics and disease management

Requirements:

  • Current LPN licensure in the state of Ohio

  • 1-2 years of experience in long-term care or a related field

  • Ability to work effectively in a fast-paced environment with minimal supervision

  • Excellent communication and interpersonal skills

  • Strong attention to detail and organizational skills

  • Ability to lift, transfer, and reposition patients as needed

  • Commitment to providing high-quality patient care and maintaining a positive and respectful work environment
