General Summary:

Munson Healthcare’s Bay Area Urology group is seeking an Advanced Practice Provider to join our collaborative team and provide high-quality, patient centered care.

The Urology Advance Practice Provider will work with Munson Urology physicians to care for patients, improve office efficiency, consultation, follow up availability, and inpatient care. This is a weekend position, offering a consistent Saturday, Sunday, Monday schedule with three 10-hour shifts (.75 FTE).

Position Duties Include:

  • Works as a member of the clinical team to promote, support and provide management of urologic medical care to patients.  
  • Evaluates and reviews the plan of care at regularly scheduled office visits utilizing sound clinical judgment based on assessment of physical, psychological, emotional, societal and environmental needs of the patients.
  • Manages patient care in the outpatient setting performing comprehensive history and physical examinations, ordering appropriate lab and radiographic studies, interpreting lab and x-ray results, performing in office procedures, managing urology medications, making treatment decisions, and determining appropriate follow up. 
  • Provides inpatient care for hospitalized patients, including admissions, daily rounds, procedures, consultations, discharge planning, and coordination of care with physicians, nursing staff, and other members of the healthcare team to ensure safe, high-quality patient outcomes.
  • Completes appropriate documentation of all evaluations, care, and services rendered in the electronic medical record to accurately reflect services in the patient record and to ensure appropriateness of billing.
  • Communicates with referring or primary care physicians regarding patient care and treatment.
  • Responds to telephone inquiries from clinic patients and provides appropriate medical care and/or counseling.
  • Assists individuals to assume responsibility for the prevention of illness and the promotion of health maintenance and/or restoration of health.
  • Observes the Munson Healthcare Advance Practice Provider prescriptive authority guidelines.
  • Maintains knowledge of current medical trends and ongoing professional development in pertinent medical fields.
  • Participates in and / or serves as a resource for medical, nursing, and community education programs.
  • Meets department productivity requirements. Recognizes and understands the need for full daily schedules.  Independently monitors own daily appointment schedule and works in conjunction with other staff members to fill open appointment times.
  • Responds to medical emergencies as required.

At High Focus Centers in Branchburg we are currently seeking a Nurse Practitioner to join our amazing team of behavioral healthcare professionals providing Partial Hospitalization (PHP) and Intensive Outpatient (IOP) treatment services to our co-occurring adult and adolescent clients. The Nurse Practitioner provides medical and/or psychiatric evaluation, assessment, medication management, care coordination, and ongoing treatment services for individuals receiving behavioral health and substance use disorder treatment services. This role collaborates with interdisciplinary treatment teams and internal and external healthcare providers to support high-quality, person-centered care in accordance with organizational, regulatory, and professional practice standards. Essential Duties and Responsibilities Complete history and physical assessments, evaluations, reassessments, and other clinical documentation as required. Review laboratory results, monitor blood levels, review physician orders, and support ongoing medical and medication management needs. Provide medication management services, including psychotropic medication management where applicable based on licensure and scope of practice. Diagnose and treat medical and/or psychiatric conditions in accordance with scope of practice and applicable regulations. Coordinate client care with internal and external healthcare providers, including primary care physicians, psychiatrists, specialists, pharmacies, and other treatment providers as appropriate. Support discharge planning and continuity of care efforts, including medication coordination and aftercare recommendations. Collaborate with nursing and clinical teams to support cost-effective medication management and completion of medication prior authorizations when needed. Provide education and clinical support to staff regarding medical, psychiatric, behavioral health, and medication-related concerns as appropriate. Participate in treatment team meetings, clinical review meetings, peer review processes, and departmental meetings as required. Maintain timely, accurate, and compliant documentation in accordance with CARF standards, licensing requirements, payer requirements, and organizational policies. Must exercise discretion and maintain confidentiality with regard to all company information Completion of all required trainings as designated by the company and accreditation/licensing entities.
