What You’ll Do

Deliver Compassionate Hospice Care

Provide direct patient care in accordance with the Nurse Practice Act, with a focus on end-of-life support
Implement individualized plans of care under the supervision of a Registered Nurse
Deliver effective pain and symptom management, monitoring and adjusting based on patient response
Support patients with daily care needs while ensuring comfort, dignity, and safety
Educate patients and families on disease progression, medications, and comfort measures

Coordinate & Communicate

Document care accurately and timely in the EMR
Communicate changes in patient condition to the RN and care team
Collaborate with interdisciplinary partners (RN, HHA, Social Work, Chaplain, Therapy)
Participate in care planning and coordination to ensure holistic support

What You Bring

Active LPN/LVN license in the state of practice
Current CPR certification
1–3+ years of nursing experience preferred (hospice, home health, or med-surg)

Communicates with community health related persons in relation to the care plan. 4. Reports suspected abandonment, abuse, financial exploitation, or neglect of a person to the department of social and health services and the proper law enforcement agency. Reports must be submitted immediately when the reporting person has reasonable cause to believe that abandonment, abuse, financial exploitation, or neglect of a vulnerable adult has occurred. 5. Teaches the patient and family/caregiver self-care techniques as appropriate. Provides medication, diet and other instructions as ordered by the physician and recognizes and utilizes opportunities for health counseling with patients and families/caregivers. Works in concert with the case manager and interdisciplinary group. 6. Provides and maintains a safe environment for the patient. 7. Assists the patient and family/caregiver and other team members in providing continuity of care. 8.
