CareCore at Mentor is seeking an experienced and motivated Registered Nurse (RN) to join our leadership team as our Assistant Director of Nursing (ADON) / Infection Preventionist.

This is a great opportunity for an RN with strong long-term care experience who enjoys being hands-on, supporting clinical staff, and maintaining high standards for infection prevention, resident care, and regulatory compliance.

Responsibilities

  • Assist the Director of Nursing with day-to-day clinical operations

  • Serve as the facility’s Infection Preventionist

  • Monitor, track, and investigate infections and infection trends

  • Maintain infection prevention and control policies, procedures, and documentation

  • Provide infection control education and training to staff

  • Assist with survey readiness and regulatory compliance

  • Support nursing staff through education, coaching, and clinical oversight

  • Participate in audits, quality improvement initiatives, and resident care reviews

  • Assist with staffing, employee performance, and clinical concerns as needed

  • Provide leadership coverage and support to the DON and nursing department

Qualifications

  • Active Ohio RN license required

  • Previous long-term care/skilled nursing experience required

  • Infection prevention/control experience strongly preferred

  • Previous nursing leadership or supervisory experience preferred

  • Strong knowledge of state and federal LTC regulations

  • PCC/PointClickCare experience preferred

  • Strong communication, organization, and leadership skills

  • Must be comfortable working collaboratively and providing hands-on support when needed

Process inquiries for all new referrals (internal and external). Ensure a seamless transition from hospital to the community setting by ordering medical supplies and DME, handing off communication with the field team and documenting in the electronic medical record. Perform assessment of potential patients consistent with Cleveland Clinic Hospice policies, procedures and admission criteria alongside the physician. Perform admission assessments for new admissions into contracted facilities and document information in the EMR within specified time frame. Plan the delivery of patient care in collaboration with other disciplines to provide comfort and symptom management. Perform daily assessment and coordinate the plan of care with inpatient staff for General Inpatients (GIP). Assess patient for safety risks, abuse and neglect. Assess and document pain using the appropriate pain scale and manage patient’s pain. Document changes in patient condition and effectiveness/timeliness of care/interventions.
