QUALIFICATIONS:

Education

· Associate Degree in Nursing (ADN) required.

· Bachelor of Science in Nursing (BSN) preferred.

Licensure

Current Texas Registered Nurse license in good standing

Experience

Minimum of three years work experience as an ICU nurse in an acute care hospital setting. – If the candidate/employee has less than three years of acute care hospital experience there will be an education plan in place required for the orientation period.

Certifications

· Basic Life Support (BLS) – required.

· Advanced Cardiac Life Support (ACLS) – required.

· NIH Stroke Scale Certification required within orientation period.

Essential Duties and Responsibilities

Clinical Care

· Performs assessment/data collection in an ongoing and systematic manner, focusing on physiologic, psychological and cognitive status of the critically ill patient.

· Formulates a goal directed plan of care, which is prioritized and based on determined nursing diagnosis and patient outcomes.

· Implements care in a knowledgeable, skilled consistent manner in reaching the desired patient outcomes through nursing processes.

· Demonstrates applied knowledge base in areas of critical care patient care, pain assessment and actions of pharmaceuticals and anesthetic agents, laboratory values, safety and legal issues and all emergency equipment.

· Monitors patient response to treatment and promptly communicates changes in condition to the provider.

· Provides care to patients across the lifespan, including pediatric, adolescent, adult, and geriatric populations as applicable.

· Assists with emergency procedures including trauma resuscitation, stroke alerts, STEMI activations, code blue responses, rapid responses, and disaster management.

· Initiates and maintains IV access and assists with advanced airway management as needed.

· Performs and interprets patient monitoring including cardiac telemetry, pulse oximetry, capnography, and hemodynamic monitoring.

· Remains flexible, adaptive and able to accept and implement new ideas and approaches.

· Treats all patients equally adopting the hospital’s Patient Rights policy.

· Reviews and carries out physician orders in an appropriate professional manner.

· Willingness to assist with other duties when asked.

Patient Throughput and Coordination

· Collaborates with physicians and multidisciplinary teams to facilitate timely patient evaluation, treatment, admission, transfer, or discharge.

· Coordinates patient care activities to support efficient patient flow and reduce length of stay.

· Ensures appropriate documentation and communication during patient handoffs.

· Participates in transfer processes and stabilization of patients requiring a higher level of care.

Documentation

· Documents assessments, interventions, patient responses, and outcomes accurately and timely within the electronic medical record.

· Maintains compliance with regulatory, legal, and accreditation requirements.

· Completes required quality and safety documentation.

Patient and Family Education

· Provides education regarding diagnoses, medications, procedures, discharge instructions, and follow-up care.

· Assesses patient understanding and readiness to learn.

· Rapid assessment, stabilization, and continuous care of critically ill patients.

· Care of patients presenting with:

o Cardiac emergencies

o Respiratory distress

o Sepsis

o Medical and surgical conditions of a critical nature requiring critical care.

· Participation in disaster preparedness and emergency management activities.

· Assists with procedural sedation and emergency procedures within scope of practice.

· Responds to hospital emergency codes as assigned.

Job Type: Full-Time Benefits Offered: Healthcare Dental Vision PTO 401K Your Job Summary The MDS Coordinator will be responsible for timely and accurate completion of both the RAI process and care management process from admission to discharge in accordance with company policy and procedures, and Federal, State and Certification guidelines, and all other entities as appropriate- Minimum Data Set, discharge and admission tracking, etc. With direction from the Director of Nursing and VP of Clinical Reimbursement, may coordinate information systems operations and education for the clinical department. Principal Responsibilities • Works in collaboration with the Interdisciplinary Team to assess the needs of the resident; Provides interdisciplinary schedule for MDS assessments and care plan reviews as required by governing agencies. • Ensures that the Interdisciplinary team makes decisions for either completing or not completing additional MDS, assessments based on clinical criteria as identified in the most recent version of the RAI User’s Manual. • Assist with coordination and management of the daily stand up meeting, to include review of resident care and the setting of the assessment reference date(s). • Complies with federal and state regulations regarding completion and coordination of the RAI process. • Monitors MDS and care plan documentation for all residents; ensures documentation is present in the medical record to support MDS coding. • Maintains current MDS status of assigned residents according to state and federal guidelines. • Maintains the frequent and accurate data entry of resident information into appropriate computerized MDS programs. • Completes accurate coding of the MDS with information obtained via medical record review as well as observation and interview with facility staff, resident and family members. • Attends interdisciplinary team meeting, quality assurance and other meeting in order to gather information, communicate changes, and maintain and update records. • Assists DON or designee with identification of a significant change, physician orders and verbal reports to assure that the MDS and care plan are reflective of those changes. • Prepares scheduling, notice of resident care planning conferences, and assists DON in communication of outcomes/problems to the responsible staff, resident, and/or responsible party. • Continually updating knowledge base related to data entry and computer technology. • Completes electronic submission of required documentation to the state database and other entities per company policy. • Corrects and ensures completion of final MDS and submits resident assessment data to the appropriate State and Federal government agencies. • Assigns, assists, and instructs staff in the RAI Process, PPS Medicare, Medicaid (Case Mix as required) and clinical computer system in relation to these processes. • Maintains confidentiality of necessary information. • Other duties, responsibilities and activities may change or assigned at any time with or without notice.
