Requirements: 

Assessment, treatment and patient care responsibilities are delivered with a knowledge of patient growth and development and are appropriate to the ages of the patients served.

Identifies and documents initial observation and assessment of the patient's needs for immediate nursing action.

Systematically and accurately assesses nursing needs of the patient in order to detect pertinent changes in the clinical picture.

Selects, plans, evaluates, and documents care based on this assessment.

Assesses, intervenes, and documents appropriately to meet patient's educational, psychosocial, and discharge planning needs.

Initiates appropriate referrals to other departments, services, and/or outside agencies for patient care.

Gives comprehensive patient care.

Initiates and follows through with patient and family teaching.

Consults with unit Nursing Leadership regarding significant changes in patient's condition.

Communicates relevant patient care information on shift-to-shift report.

Utilizes patient care standards in clinical practice.

Collaborate with all members of the health care team.
Documents pertinent information regarding patient's response to treatment and progression toward goals in patient's chart.

Completes Admission/Transfer Summary Sheet and/or Discharge Summary.

Transcribes physician's orders in patient charts accurately and timely (when Ward Clerk is not available).

Seeks clarification of physician's orders that are unclear or inappropriate.

Interprets clinical data and notifies the physician promptly if immediate changes in management are indicated.

Administers intravenous medications in compliance with policies and procedures.

Titrates medicated infusions as per physician's prescriptive, protocols, and standard. Transports patients to operating room and from post anesthesia care unit.

Checks to ensure that emergency equipment is on hand and in good working order.

Recognizes and reports malfunction or failure of equipment used in the area to Biomedical Engineering; takes appropriate action to repair/replace such equipment (e.g., ventilators, cardiac monitors, defibrillators, suction apparatus, pumps, doppler, O2 tank, and pacemaker).

Checks policy and procedures on proper handling of sharps.

INDIRECT PATIENT CARE ACTIVITIES
Participates in interdisciplinary patient care rounds by presenting findings on patients.

Apply principles of infection control and safety.

Assists in the orientation of new personnel.

Provides clinical supervision of LPNs, Certified Nursing Assistant, Ward Clerks and other ancillary staff.

Customer Service: respect, flexibility, knowledge, confidence, professionalism, pleasant attitude, patience and helpfulness. All responses should be timely, professional, caring, and respectful in accordance with Customer Service Performance expectations

Maintains established departmental policies and procedures, objectives, quality assurance program, safety, environmental, and infection control standards. Assesses gaps in policies and procedures and create necessary policies and procedures to fulfill these gaps.

Position Qualifications:
Current NJ licensure as RN required.
BLS & IV certification required.
Good oral and written communication skills.
Good interpersonal skills.
Speaks, reads and writes English to the extent required by the position.

Education:

BSN required, or proof of enrollment in an accredited BSN program with completion expected within three (3) years of start date.

Manages and coordinates all aspects of assigned caseload. • Assists patients with activities of daily living, which may require positioning, moving, transferring and lifting patients of varying weights and physical conditions, with and without assistance and as appropriate, from family members and/or paraprofessionals. • For NFP Only • Establishes, implements, and revises a plan of care with input from the client and from appropriate multidisciplinary health team members (i.e., social worker, physician) based on goals mutually acceptable to the client. Ensures that services follow the plan of care by ongoing assessment, planning, coordination, counseling, teaching, and evaluation of the delivery of care to meet the needs of the client. • Manages, plans, coordinates and executes the client’s safe discharge from service. Ensures that discharge summary and communication to team members, patient and family members adhere to VNS Health and regulatory requirements. • Maintains a flexible work schedule to meet clients needs. • Participates in all required NFP training sessions. • Participates in special projects and performs other duties as assigned. Qualifications Licenses and Certifications: License and current registration to practice as a registered professional nurse in New York State required Valid driver's license or NYS Non-Driver photo ID card, may be required as determined by operational/regional needs. For Hospice Only: Wound Care Certification required or willingness to obtain within one (1) year of hire preferred Certification in Hospice and Palliative Care Nursing preferred Education: Degree in Nursing from an approved program required Bachelor's Degree NFP Only- Nursing required Work Experience: Experience for CHHA: Minimum of one year of experience in a medical/surgical environment required Experience for Hospice: Minimum of one year of clinical nursing experience, including at least six months of Hospice, or Med/Surg or ICU or Emergency Dep’t or Home care, Palliative Care, Oncology or Long Term Care experience required Experience for NFP: Minimum of one year of recent experience in maternal/child health required Demonstrated basic computer skills required Pay Range per Visit: If you are applying to the per diem per visit version of this job, the hiring range is as follows: $78.64- $123.20 per visit.
