Position Overview:

As a Nurse Practitioner, you will play a pivotal role in providing primary care services to patients across multiple sites, collaborating closely with our Medical Director and Director of Medical Practice Operations. The Nurse Practitioner role is a vital position in the Monadnock Health Partners (MHP) network of primary care practices. Patient Care Responsibilities:

  • Patient-Centered Care: Conduct thorough assessments and collaborate on personalized care plans.
  • Comprehensive Treatment: Diagnose and treat illnesses, prescribing medications as needed.
