About the Role

We are seeking a compassionate and dependable Registered Nurse (RN) – Overnight Hospice Care to join our hospice team full-time.

This position is dedicated exclusively to overnight care, providing clinical support and urgent hospice services during the night hours. The schedule consists of 8-hour overnight shifts and includes working every other weekend.

In this role, you will respond to patient and family needs overnight, complete nursing assessments, provide direct hospice care, and collaborate with the interdisciplinary team to ensure patients receive exceptional care and support when they need it most.

This position travels throughout the entire Blue Ridge Care service area.

Schedule

  • Full-time, overnight shifts only

  • 8-hour shifts

  • Every other weekend required

  • Eligible for applicable shift differentials

How You'll Make a Difference

  • Respond to emergent calls from patients and families during overnight hours.

  • Provide overnight visits to address urgent concerns, symptom changes, or end-of-life needs.

  • Deliver skilled nursing care in accordance with patient goals and physician orders.

  • Assess and evaluate patient conditions and provide clinical oversight when LPN support is involved.

  • Develop, update, and implement individualized plans of care.

  • Administer medications and treatments as prescribed.

  • Support symptom management and patient comfort throughout the night.

  • Educate patients and families on disease progression, symptom management, and care techniques.

  • Provide emotional support and reassurance during difficult and often unexpected situations.

  • Communicate significant changes in condition to physicians and obtain necessary orders.

  • Complete timely and accurate clinical documentation.

  • Coordinate equipment, supplies, and services needed to support quality care in the home.

  • Collaborate with the interdisciplinary team to ensure continuity of care.

  • Assist with admissions, crisis visits, death visits, and other overnight hospice needs as appropriate.

  • Participate in continuing education, quality improvement activities, and team meetings.

What You'll Bring

  • Current Virginia Registered Nurse (RN) license.

  • Graduate of an accredited nursing program; BSN preferred.

  • Two or more years of hospice, home health, ICU, critical care, or related nursing experience preferred.

  • CHPN certification preferred.

  • Current CPR certification.

  • Strong assessment, critical thinking, and clinical decision-making skills.

  • Knowledge of Medicare and Medicaid hospice regulations.

  • Proficiency with electronic medical records.

  • Ability to travel throughout the Blue Ridge Care territory.

  • A calm, confident presence during urgent and emotionally challenging situations.

  • Ability to work independently during overnight hours while collaborating effectively with the broader care team.
