About the Role
We are seeking a compassionate and dependable Registered Nurse (RN) – Evening Hospice Care to join our hospice team full-time.
This position is dedicated to providing evening hospice care and support, ensuring patients and families have access to clinical expertise during some of the most critical hours of the day. The schedule may consist of either 8-hour or 10-hour evening shifts and includes working every other weekend.
In this role, you will respond to patient and family needs, complete nursing assessments, provide direct hospice care, and collaborate with the interdisciplinary team to ensure continuity of care. You will serve as a trusted resource, helping patients remain comfortable and supported while guiding families through challenging situations.
This position travels throughout the entire Blue Ridge Care service area.
Schedule
Full-time, evening shifts
8-hour or 10-hour shift options available
Every other weekend required
Eligible for applicable shift differentials
How You'll Make a Difference
Respond to emergent calls from patients and families during evening hours.
Provide visits to address urgent concerns, symptom changes, or end-of-life needs.
Deliver skilled nursing care in accordance with patient goals and physician orders.
Assess and evaluate patient conditions and provide clinical oversight when LPN support is involved.
Develop, update, and implement individualized plans of care.
Administer medications and treatments as prescribed.
Support symptom management and patient comfort.
Educate patients and families on disease progression, symptom management, and care techniques.
Provide emotional support and reassurance during difficult situations.
Communicate significant changes in condition to physicians and obtain necessary orders.
Complete timely and accurate clinical documentation.
Coordinate equipment, supplies, and services needed to support quality care in the home.
Collaborate with the interdisciplinary team to ensure continuity of care.
Assist with admissions, crisis visits, death visits, and other after-hours hospice needs as appropriate.
Participate in continuing education, quality improvement activities, and team meetings.
What You'll Bring
Current Virginia Registered Nurse (RN) license.
Graduate of an accredited nursing program; BSN preferred.
Two or more years of hospice, home health, ICU, critical care, or related nursing experience preferred.
CHPN certification preferred.
Current CPR certification.
Strong assessment, critical thinking, and clinical decision-making skills.
Knowledge of Medicare and Medicaid hospice regulations.
Proficiency with electronic medical records.
Ability to travel throughout the Blue Ridge Care territory.
A calm, confident presence during urgent and emotionally challenging situations.
Ability to work independently while collaborating effectively with the broader care team.
