Service Area: Schoharie County and surrounding counties, including Montgomery, Schenectady, Albany, Greene, Otsego, Delaware, Fulton, Saratoga, Rensselaer, Washington, Warren, and Columbia Counties. Additional service areas may be available based on patient needs.

Job Summary

At Home Care Partners, we are seeking a compassionate and experienced Registered Nurse (RN) to join our home care team serving patients throughout Schoharie County, New York.

This is a full-time field-based nursing position for an RN who enjoys working independently while providing personalized care in patients' homes and community-based settings.

The Home Care Field RN is responsible for conducting in-home nursing assessments, developing and updating Plans of Care, completing home health aide supervisions, providing clinical guidance, and helping ensure safe, high-quality patient care.

Responsibilities

  • Travel to patient homes and community-based settings throughout Schoharie County and assigned service areas

  • Conduct comprehensive in-home nursing assessments

  • Perform head-to-toe patient evaluations, including medication and diagnosis reviews

  • Develop, review, and update individualized Plans of Care (POC)

  • Complete required home health aide supervisions

  • Provide clinical guidance and support to home health aides and caregivers

  • Orient aides to each patient's Plan of Care and individual care needs

  • Monitor patients for changes in condition and communicate concerns appropriately

  • Collaborate with patients, families, caregivers, home health aides, and office staff

  • Complete accurate and timely clinical documentation

  • Help maintain continuity, safety, and quality of patient care

  • Follow applicable healthcare regulations, clinical standards, and company policies

Qualifications

  • Current and active New York State Registered Nurse (RN) license

  • Ability and willingness to travel throughout Schoharie County and assigned service areas

  • Medical Professional Liability insurance

  • Strong clinical assessment and critical-thinking skills

  • Excellent written and verbal communication skills

  • Strong organizational and clinical documentation skills

  • Comfortable using Gmail and Microsoft Word/Excel

  • Ability to work independently and manage a field-based schedule

  • Professional, dependable, compassionate, and patient-focused

  • Must meet all company hiring and onboarding requirements

Preferred Experience

  • Home care or home health nursing

  • Community health or outpatient nursing

  • In-home patient assessments

  • Home health aide supervision

  • Developing, reviewing, or updating Plans of Care

  • Experience with Medicaid, Medicare, or MLTC plans

  • Spanish/English bilingual skills are a plus
