Job Summary The RN - Nurse Navigator acts as the primary point of contact and is responsible for monitoring and coordinating care for cancer patients and families. Individualized assistance offered to patients, families, and caregivers to help overcome healthcare system barriers and facilitate timely access to quality health and psychosocial care from pre-diagnosis through all phases of the cancer Core Job Responsibilities Act a single point of contact for patients and their families. Facilitates appointments for consults and support services within establish service standards. Provide information throughout the continuum of care including educating the patient, their family, and caregivers regarding their diagnosis and treatment options. Assists patients in understanding their diagnosis, treatment options, and the resources available, including educating eligible patients about appropriate clinical trials. Duties Description The RN 1 serves as a member of the interdisciplinary team performing various nursing duties, including coordinating and monitoring all aspects of medical care for residents. Duties And Responsibilities Include But Are Not Limited To Provide direct nursing care to residents, including comprehensive nursing assessments, administering medications, implementing physician’s treatment orders, transcribing orders, and collecting specimens for laboratory analysis or culture. The RN Coordinator of the ED/ICU is a Registered Nurse (RN) who has experience in nursing practice with demonstrated leadership and management skills in the acute care hospital Emergency/ICU specialty.
