✨ What You'll Do

As an RN Case Manager, you'll be the heart of the care team, serving as a trusted advocate for patients and families while coordinating personalized hospice services.

In this role, you will:

✅ Assess patient and family needs and develop individualized care plans

✅ Deliver skilled nursing care focused on comfort, symptom management, and quality of life

✅ Coordinate care among physicians, hospice aides, social workers, chaplains, and other team members

✅ Educate and support families through every stage of the hospice journey

✅ Supervise and guide hospice aides to ensure exceptional patient care

✅ Maintain accurate clinical documentation and participate in ongoing professional development

What You'll Bring

We're looking for a nurse who combines strong clinical skills with a servant's heart.

Qualifications:

✔ Current Virginia RN license (single-state or multistate)

✔ CPR certification

✔ Minimum two years of nursing experience

✔ Hospice, palliative care, or home health experience preferred

✔ Strong communication, assessment, and organizational skills

✔ Valid driver's license, reliable transportation, and automobile insurance

✔ A compassionate, self-motivated approach to patient-centered care

Provide visits to address urgent concerns, symptom changes, or end-of-life needs. Deliver skilled nursing care in accordance with patient goals and physician orders. Assess and evaluate patient conditions and provide clinical oversight when LPN support is involved. Develop, update, and implement individualized plans of care. Administer medications and treatments as prescribed. Support symptom management and patient comfort. Educate patients and families on disease progression, symptom management, and care techniques. Provide emotional support and reassurance during difficult situations. Communicate significant changes in condition to physicians and obtain necessary orders. Complete timely and accurate clinical documentation. Coordinate equipment, supplies, and services needed to support quality care in the home. Collaborate with the interdisciplinary team to ensure continuity of care. Assist with admissions, crisis visits, death visits, and other after-hours hospice needs as appropriate. Participate in continuing education, quality improvement activities, and team meetings. What You'll Bring Current Virginia Registered Nurse (RN) license. Graduate of an accredited nursing program; BSN preferred. Two or more years of hospice, home health, ICU, critical care, or related nursing experience preferred. CHPN certification preferred.
