Monitors and maintains unit compliance with regulatory, accrediting and hospital policy for patient services, and environmental and personnel safety.

Manages the resolution of issues that arise during the course of the shift. Executes decisions in the absence of the hospital administration. Determines need to contact Administrator on Call. Coordinates with Chief Nursing Officer and unit managers as indicated.
Evaluates issues and identifies trends. Communicates trends to hospital leadership and collaborates on problem solving to address trended issues.
Serves as the leader of the hospital safety plan in the absence of hospital administrators and other leaders. Includes coordinating the response to reported incidents including employee injuries.
Monitors and evaluates clinical standards within the Hospital.
Serves as clinical support for Emergency Department, Inpatient and ICU as needed (may require direct patient care assignment).
Ensures that age and culturally appropriate care is given.
Follow Stand Precautions using personal protective equipment as required for procedures.

Principal Responsibilities • Works in collaboration with the Interdisciplinary Team to assess the needs of the resident; Provides interdisciplinary schedule for MDS assessments and care plan reviews as required by governing agencies. • Ensures that the Interdisciplinary team makes decisions for either completing or not completing additional MDS, assessments based on clinical criteria as identified in the most recent version of the RAI User’s Manual. • Assist with coordination and management of the daily stand up meeting, to include review of resident care and the setting of the assessment reference date(s). • Complies with federal and state regulations regarding completion and coordination of the RAI process. • Monitors MDS and care plan documentation for all residents; ensures documentation is present in the medical record to support MDS coding. • Maintains current MDS status of assigned residents according to state and federal guidelines. • Maintains the frequent and accurate data entry of resident information into appropriate computerized MDS programs. • Completes accurate coding of the MDS with information obtained via medical record review as well as observation and interview with facility staff, resident and family members. • Attends interdisciplinary team meeting, quality assurance and other meeting in order to gather information, communicate changes, and maintain and update records. • Assists DON or designee with identification of a significant change, physician orders and verbal reports to assure that the MDS and care plan are reflective of those changes. • Prepares scheduling, notice of resident care planning conferences, and assists DON in communication of outcomes/problems to the responsible staff, resident, and/or responsible party. • Continually updating knowledge base related to data entry and computer technology. • Completes electronic submission of required documentation to the state database and other entities per company policy. • Corrects and ensures completion of final MDS and submits resident assessment data to the appropriate State and Federal government agencies. • Assigns, assists, and instructs staff in the RAI Process, PPS Medicare, Medicaid (Case Mix as required) and clinical computer system in relation to these processes. • Maintains confidentiality of necessary information. • Other duties, responsibilities and activities may change or assigned at any time with or without notice. Qualifications • Graduate of an approved Registered Nurse program and licensed in the state of practice, required. • Minimum of 2 years of nursing experience in a Skilled Nursing Facility preferred. • Excellent knowledge of Case-Mix, the Federal Medicare PPS process, and Medicaid reimbursement, as required. • Thorough understanding of the Quality Indicator process. Knowledge of the OBRA regulations and Minimum Data Set. • Knowledge of the care planning process. • Experience with MDS 3.0, preferred. Responsibilities The Emergency Department (ED) Charge Nurse is responsible and accountable to the Department Director for the quality of nursing staff and the delivery of patient care during his/her shift and assists the Director in the daily operation of the respective unit(s)/department. He/she provides direct communication with the Director and/or House Supervisor of unanticipated� patient care events and/or risk management activities within patient care unit(s); Utilizes nursing personnel effectively, assisting with required scheduling, staffing and assignments; Provides direction, supervision and evaluation of staff and/or staff education as necessary; Assists with performance evaluations, progressive discipline of staff and other related HR activities as needed; Assesses supplies and equipment availability, ensures their proper use and functionality and assists in the daily maintenance of par levels; Collaborates with other department managers/supervisors to ensure smooth coordination of services to patients; enhanced patient flow within the facility and efficient admission/discharge/transfer processes; Assesses patient care delivery and quality of care/documentation on a consistent basis; directs staff members to make immediate changes in the plan of care, as appropriate to ensure patient safety and adherence to regulatory guidelines, hospital policies, procedures, protocols and/or clinical pathways and notifies Department Director, House Supervisor and/or Medical Staff as appropriate utilizing the chain of command. Qualifications Education and Work Experience Current and valid state RN License. Current BLS (AHA) certificate upon hire and maintain current. Current ACLS (AHA) certificate 30 days upon hire and maintain current. Current PALS (AHA) certificate and/or ENPC required 30 days upon hire and maintain current. Previous supervisory experience, preferred. Minimum of one year as a staff nurse in an acute care hospital setting, E.D. setting preferred. Trauma Nurse Core Course (TNCC) certificate within 12 months of hire and Certified Emergency Nurse (CEN) within 2 years of hire, preferred. Bachelor's degree in Nursing (BSN) preferred.
