As a Registered Nurse Behavioral Health, you’ll contribute to our success in the following ways: 

  • Assess patients' physical, emotional, social, and environmental needs. 
  • Manage a caseload and develop, coordinate, and update individualized care plans. 
  • Complete initial assessments and provide ongoing evaluations during patient visits. 
  • Monitor patient progress and communicate changes with physicians, supervisors, and the care team. 
  • Update care plans based on patients' changing needs. 
  • Educate patients, families, and caregivers about the plan of care and behavioral health support. 
  • Teach behavioral health care and symptom management to staff in partner care settings. 
  • Supervise and support Behavioral Health Aides and LPNs while ensuring quality care. 
  • Report safety concerns or other issues to supervisors as needed. 
  • Provide compassionate end-of-life support and complete bereavement assessments. 

What is Required? 

  • Current State license as a Registered Nurse.  
  • Graduate of an accredited Certificate, Diploma, Associates or Baccalaureate School of Nursing.  
  • 1+ year of experience as a nurse in a clinical care setting.  
  • Home Health experience is preferred, but not required.   
  • Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment   
  • Must possess a valid driver's license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations.
