Looking for an experienced MDS RN or LPN with at least 1 year or more MDS experience (required) and preferably that experience includes some time during the last 2 years.

The MDS Coordinator's responsibilities include:

  • Provide accurate documentation of all services in order to assure accurate billing and reimbursement from all payor sources.

  • Coordinate the development of a written care plan and assessment for each resident and review and revise as appropriate.

  • Provide education to floor staff how to complete accurate documentation

Requirements:

  • Must be a state-licensed LPN or RN

  • Must have 1 - 2 years MDS and Point Click Care experience

  • Preferably have experience with PDPM
