Clinical Assessment Coordinator of Behavioral Health (Nursing/RN Registered, LPC, or LMSW) – Eagle Pass, TX

 

Horizon Health is seeking a Clinical Assessment Coordinator (Nursing/RN Registered, LPC, or LMSW) to oversee the Behavioral Health Fort Duncan Hospital in Eagle Pass, TX. This inpatient psychiatric unit has a total of 18 inpatient psychiatric beds (Adults & Seniors). The Clinical Assessment Coordinator completes the referral development and pre-admission patient assessment process and provide clinical assessments for potential clients. A MA/MSW Clinical Assessment Coordinator will complete specialized clinical evaluations, diagnose behavioral health conditions and develop moderate to complex treatment plans. 

 

Responsibilities:

  • Diagnose, implements, coordinates and evaluates clinical admission, referral and/or discharge processes.
  • Conducts on and off-site patient assessments, including appropriate test administration, interpretation and recommendations.  Responds to intake calls and completes admission paperwork.
  • Participates in the development of treatment plans in coordination with the Treatment team. Coordinates treatment with physicians and other health and social agencies and participates as an active member of the interdisciplinary team.
  • Utilizes assessment tools including, but not limited to Geriatric Depression Scale, Zung, LUMS, alcohol screening tool(s) under authorization/supervision of RN, LPC or LMSW.
  • Communicates with referral sources including family members, legal system, psychiatrists, primary care physicians, and other treatment facilities.
  • Reviews newly admitted patient accounts for eligibility and authorization daily.
  • Maintains schedule for filing court paperwork and hearings. Works closely with court personnel to maintain good relationships.
  • Enters required data into HorizonPlus system and generates monthly reports to track and trend referral, admission and discharge data. 
  • Performs other duties as assigned/required by this position.

Qualifications

In addition, this position performs clinical admission duties, including intake assessment, triage, as well as coordinating efficient workflows. The position also serves as a behavioral health resource to internal hospital departments by providing education, clinical guidance, and support related to behavioral health services and admission processes. This position requires an active Registered Nurse license and experience conducting behavioral health assessments, evaluating patients for admission, and applying behavioral health admission criteria. The work schedule includes evening hours and participation in after-hours on-call coverage. Responsibilities Registered Nurse (RN) - Day Shift from 7am-7pm (Full-Time) – Murray, KY Horizon Health together with Murray Calloway is seeking Registered Nurse (RN) - Day Shift (Full-time) for the inpatient Behavioral Health unit in Murray, KY. This is a 12 bed geriatric psychiatric unit . A registered psychiatric nurse (RN) executes comprehensive nursing care activities. This position is responsible for assessing, monitoring, recording and reporting symptoms or changes in patient conditions. RNs are required to maintain accurate patient reports and medical histories, perform mental and physical assessments, administer medication and treatment to patients, provide assistance to patients with daily activities and observe reactions. They are also responsible for managing activities with the therapeutic milieu including daily groups, education, 1:1 with individual patients and collaborating with other members of the treatment team to insure the best possible care for the patients. Responsibilities: Complete initial nursing assessment with 24 hours of admission on per facility policy; reassess routinely throughout patients’ stay at the facility Complete individualized treatment plans for each patient Plan, implement and coordinate nursing care activities for patients. Conduct therapies in accordance with patient’s diagnosis and needs. Perform treatment intervention following physician orders such as medication administration and medical treatment orders. Medication reconciliation Follow voluntary and involuntary procedure/process. Interpret the nursing and treatment needs of the patient to nonclinical staff assigned to the patient. Insure patient dignity and respect patient values. Establish a compassionate environment by providing emotional and psychological support to patients, families and friends. Performs other duties as assigned/required by this position.
