Responsibilities Nurse Manager- Inpatient Psychiatric Unit Horizon Health is seeking a Nurse Manager to join our team at Abbeville General Hospital in Abbeville, LA. This is a 16 bed inpatient psychiatric unit treating ages 18+. The Nurse Manager plans, implements, and evaluates delivery of overall nursing care to patients. He/she provides managerial support in conjunction with the clinical leadership by providing supervision and evaluation of all nursing and patient care personnel. He/she contributes to the facility through commitment to nursing goals, the supervision of clinical staff, participation on the treatment team(s). Responsibilities: Create, implement and enforce clinical staffing plan for direct reports ensuring sufficient nursing personnel to provide for patient care needs in keeping with appropriate regulatory requirement Provide leadership and direction in accordance with the client hospital guidelines and consistent with nursing and Program goal philosophies Provide for professional growth and development of nursing staff through identification of needs, development of individual training plans, participation in training programs, encouragement of formal continuing education efforts, and evaluation. Monitor nursing personnel performance on a continuous basis and conduct regular performance evaluations for clinical direct reports. Collaborate with physicians, multidisciplinary team members, and outside health and social agencies. Ensure efficient discharge plans and accurate histories are obtained at admission. Ensure the completion of all required documentation relevant to patients in a timely, efficient manner. Continuously audit and review performance and documentation of direct reports for consistency and accuracy. Assure quality of care by adhering to therapeutic standards, measuring health outcomes against patient care goals and regulatory standards Maintain continuity among nursing teams by documenting and communicating Performs the duties of an RN as needed or when there is a shortage in staff. Orient or mentor new nursing staff Performs other duties as assigned/required by this position. Responsibilities Nurse Practitioner (PRN) - Bay Minette, AL . ***PRN – as needed- at minimum one weekend a month. Shifts could be 8-12 hour shifts based on needs *** Horizon Health Corporation is seeking a Nurse Practitioner (PRN) for an inpatient geriatric psychiatric program in Bay Minette, AL . This unit is a 15 beds mental health program. Under the general supervision of the Medical Director, the Nurse Practitioner (PRN) will provide clinical services to the behavioral health program and is primarily responsible for diagnosing and treating patients; and providing continuing education to patients and their families according to approved protocols. The NP will assess the patients by examining medical history, physical exams and behavioral health evaluations; and ordering or performing diagnostic tests, and within the bounds of scope of practice as delineated by both federal and state regulations/guidelines. Responsibilities: Complete patient diagnoses, treatment, education and referrals in accordance with Hospital policies Perform treatment planning and intervention Medication orders and reconciliation Work with the treatment team in admitting and discharging patients to and from the Program Conduct physical exams of the patient during admission and attend to basic medical needs/issues; work with hospital specialists to manage more complicated medical conditions Perform admission work-ups within twenty-four (24) hours of admissions and provide appropriate documentation as may be defined by Hospital policies Conduct rounds on patients Perform as part of the treatment team in planning, coordinating and implementing nursing care activities for patients Ensure optimal patient care Assist Program leadership in assessing, creating and implementing advanced practice provider procedures, or ensuring existing procedures meet federal and state regulations and guidelines for Nurse Practitioner scope of practice Performs other duties as assigned/required by this position.
