Position Description:
 

The Registered Nurse in the Wellness for Life Department provides clinical nursing services across community, employer, mobile, and other offsite healthcare settings throughout Terrebonne General Health System’s service area. Under the direction of the Director of Employer and Consumer Health, the RN supports initiatives designed to improve access to care, promote prevention and early identification of health risks, provide patient education, and connect individuals with appropriate healthcare services and community resources. This position requires the ability to function independently in offsite and community-based settings while maintaining communication and collaboration with providers, departmental leadership, and other members of the health care.

Qualifications:
1. Active and unrestricted Registered Nurse (RN) license in Louisiana.
2. Minimum of (2) years of nursing experience preferred, preferably in wellness or community health.
3. Current Basic Life Support (BLS) certification required.
4. Experience in community health, ambulatory care, primary care, wellness, population health, occupational health, or other outpatient setting preferred.
5. Experience with Epic preferred.
6. Strong clinical judgement and ability to recognize changes in patient condition and respond appropriately according to established protocols and scope of practice.
7. Excellent communication and interpersonal skills.
8. Ability to adapt to changing clinical, operational, and community needs and manage multiple responsibilities and priorities.
9. Demonstrates leadership skills in patient education and community outreach.
10. This position requires the ability to operate a vehicle as a job-related function; candidates must have a valid driver’s license and a motor vehicle record acceptable under company and insurance guidelines.
