Well-staffed Nursing and Rehabilitation Center
3pm-11pm


JOB DESCRIPTION:

The Certified Nursing Assistant works directly under the supervision of the Registered Nurse or Licensed Practical Nurse (LPN) and performs duties in a relation to the personal needs of the resident, care of supplies and equipment, and maintaining the cleanliness, comfort and safety of the resident in his/her environment.

REPORTS TO: Charge Nurse, Supervisor and Director of Nursing Services (DNS)

  • Ability to comprehend and follow verbal and written instructions.
  • Ability to write legibly.
  • Physically able to perform the required duties of a Certified Nursing Assistant.
  • Experience as a Nursing Assistant in an acute care or long-term care setting.


RESPONSIBILITIES: Residents

  • Provides or assists with bed bath, tub bath, whirlpool and showers. (Supervises safety of same.)
  • Provides personal grooming: i.e. hair, nails, shaving, personal clothing, mouth care, perineal care.
  • Prepares laundry for pick-up and return.
  • Checks resident's body for skin care: i.e. rash, any discoloration or edema.
  • Feeding and Assisting with Feeding (when necessary)
  • Provides calm, pleasant environment.
  • Engages in cluster feeding while seated.
  • Monitors daily intake and keeps nurses informed.
  • Records food intake on Nutrition Work Sheet.
  • Making Beds
  • Uses recommended amount of linen and diapers.
  • Disposes of linen in proper manner.
  • Properly utilizes clean and dirty linen carts.
  • Secures call bells to bed.
  • Beds are made in proper manner.
  • Knowledgeable in procedure for linen precaution.


Safety of Residents:

  • Restraints are to be properly applied and released and removed every two (2) hours for at least 15 minutes.
  • Make sure, if restraint is used, that it was indicated on assignment and the proper type is applied.
  • Restraints are checked at the beginning and end of each shift.
  • Supervises ambulation of resident when needed.
  • Proper attention is given that side rails are up on bed when needed.
  • Completes dining room duty as posted by charge nurse.
  • Is alert for spills on floor.
  • Answers call bells as soon as possible.
  • Makes rounds frequently.
  • Reports need for repair and safety matters to charge nurse.

To Ascertain Vital Signs

  • Follows procedure in taking TPR correctly.
  • Adheres to the proper procedure for cleaning thermometers/IVAC.

Rehabilitation

  • Assists in ambulation, transfers, ROM exercises and good body alignment where indicated.
  • Uses adaptive devices properly.
  • Cooperates with all rehab disciplines.
  • Keeps accountability sheets up to date with signatures as required.

Testing of Urine and Collecting of Specimens

  • Has knowledge of proper procedure.
  • Records results.
  • Informs nurse of results.

Assignments

  • Receives daily report from nurse.
  • Consults assignment for special duties and sets proper priorities for following through on assignment.
  • Is familiar with and takes proper action in meeting physical needs of resident.
  • Routinely checks and keeps in order all residents' belongings: i.e. clothing, glasses, hearing aid, prosthesis aid, adaptive devices, etc.
  • Ensures that residents on turning schedule is positioned every two hours or as ordered, and documented.
  • Ensures that residents on toileting schedule are assisted as needed, and documented.
  • Resident is kept clean and dry.
  • Gives assistance to co-workers when needed.
  • Uses good handwashing techniques.

Cooperation

  • Works well with co-workers and supervisors.
  • Maintains appropriate relationship with residents, families and peers.
  • Gives assistance to other departments: i.e. recreation, when necessary.
  • Communicates well with other staff members.

License:

  • Certified Nursing Assistant (CNA) (Required)
  • CPR (Preferred)
